Abstract: | Fibrolamelārā hepatocelulārā karcinoma (FLHCC) ir rets primārs ļaundabīgs aknu audzējs, kas parasti rodas jauniem cilvēkiem bez iepriekšējas aknu patoloģijas. Lai gan kopumā reti sastopama, FLHCC izraisa ievērojamu primāro aknu vēža gadījumu skaitu pacientiem, kas jaunāki par 40 gadiem. Sākotnējā diagnoze bieži vien balstās uz attēldiagnostiku un klīniskajām pazīmēm, bet zelta standarts joprojām ir biopsijas materiāla vai ķirurģiski rezecēto audzēju patoloģiskā izmeklēšana. FLHCC nereti tiek diagnosticēts vēlīnā stadijā, un recidīvi attīstās bieži. Operācija rezekcijas vai aknu transplantācijas veidā un atkārtota recidīvu operācija ir vienīgās potenciāli radikālās ārstēšanas iespējas. Adjuvantas un neoadjuvantas terapijas ietekme uz kopējo dzīvildzi joprojām nav skaidra. Kopējā dzīvildze ir līdzīga hepatocellulāras karcinomas pacientiem bez aknu cirozes. Šī literatūras apskata mērķis ir sniegt visaptverošu pārskatu par FLHCC patoloģiskajām, ģenētiskajām un radioloģiskajām iezīmēm un izvērtēt slimību modeļus un dažādu ārstēšanas veidu ietekmi uz kopējo dzīvildzi. Fibrolamellar hepatocellular carcinoma (FLHCC) is a rare type of primary malignant liver tumour that usually occurs in young individuals with otherwise healthy livers. Although generally rare, FLHCC accounts for a significant amount of primary liver cancers in patients under 40 years old. The first diagnosis is often based on imaging and clinical features, but the gold standard remains pathological examination of biopsy material or resected tumors. FLHCC is often diagnosed at an advanced stage and recurrences are common. Surgery in form of resection or liver transplantation and repeat surgery of recurrences are the only potentially curable treatment options. The effect of adjuvant and neoadjuvant treatment on overall survival remains unclear. Overall survival is similar to that of HCC in non-cirrhotic liver. The aim of this literature review is to give the reader a comprehensive account of the pathologic, genetic and imaging features of FLHCC and shine a light on disease patterns and the effect of different types of treatment on overall survival. |
