Abstract

Zinātniskajā literatūrā secināts, ka šobrīd ir ļoti maz randomizētu pētījumu par izgulējumu ārstēšanas programmu ārstēšanas rezultātiem un izmakām. Atzīst, ka ir nepieciešami jauni, plaši pētījumi, lai izvērtētu nepieciešamos fiziskos un ekonomiskos resursus šo programmu īstenošanai. Darba autore izvirzīja mērķi - novērtēt izgulējumu, tai skaitā komplicētu ar osteomielītu un ilgstoši nedzīstošu, hronisku mīksto audu čūlu (problēmbrūču) mikroķirurģiskās ārstēšanas, programmas ārstēšanas rezultātus Ķirurģiskās infekcijas klīnikā un sniegt priekšlikumus ārstēšanasprogrammas efektivitātes paaugstināšanai. Pētījuma mērķa sasniegšanai autore izpētījusi zinātnisko literatūru un citu valstu pieredzi izgulējumu un komplicētu brūču ārstēšanā, veikusi izgulējumu programmas īstenošanas kārtības, pakalpojumu spektra un apjomu izpēti Latvijā, novērtējusi izgulējumu un komplicētu brūču ārstēšanas programmas rezultātu, efektivitāti un izmaksas, uz kā pamata izstrādāja priekšlikumus izmaksu efektīvas pieejas sekmēšanai un pakalpojuma attīstībai. Pētījuma rezultātā izpētīta Izgulējumu un komplicētu brūču ārstēšanas programmas organizācija un veikta programmas novērtēšana tās efektivitātes uz izmaksu kontekstā. ekonomisko izmaksu analīze, izstrādātas rekomendācijas izmaksu efektīvas pieejas sekmēšanai un pakalpojuma attīstībai. Novērtējums liecina, ka vairākos rādītājos efektivitāte ir līdzvērtīga citu valstu programmas efektivitātei, taču izmaksas ir ļoti augstas un valsts noteiktā apmaksa neatbilst reāli nepieciešamajām manipulācijām un ārstniecības iestādes izmaksām. Autore piedāvā pārskatīt programmas organizāciju, izstrādājusi divus alternatīvus programmas algoritmus, kā rezultātā izstrādātas rekomendācijas, pacientu vajadzībām un aprūpes procesam atbilstošākas un izmaksu ziņā efektīvākas pieejas īstenošanai. Pētījumam ir zinātniskā novitāte un tā rezultātiem – praktiskais pielietojums. Autore piedāvā metodiku programmas izmaksu un efektivitātes novērtēšanai un pirmo reizi Latvijā tika izvērtēti izgulējumu ārstēšanas programmas rezultāti, aprēķinātas izmaksas un izstrādātas rekomendācijas programmas tālākai attīstībai. The scientific literature concludes that there are currently very few randomized trials on the treatment outcomes and costs of decubitus treatment programs. It is recognized that new, extensive research is needed to assess the necessary physical and economic resources for the implementation of these programs. The author of the paper set the goal - to evaluate the results and costs of the microsurgical treatment program for decubitus, including complicated wounds with osteomyelitis and long-term non-healing, chronic soft tissue ulcers in the Surgical Infection clinic and to provide proposals for increasing the effectiveness of the treatment program. To achieve the goal of the research, the author has studied the scientific literature based on the experience of other countries in the treatment of decubitus and complicated wounds, she has carried out a study of the procedures for the implementation of the decubitus program, the spectrum and scope of services in Latvia, she has evaluated the results, efficiency and costs of the treatment program for decubitus and complicated wounds, on the basis of which she developed proposals for cost-effective approach to promotion and service development. As a result of the study, the organization of the program for the treatment of decubitus and complicated wounds was investigated and the program was evaluated in the context of its cost effectiveness. Analysis of economic costs developed recommendations for the promotion of a cost-effective approach and development of the service. The evaluation and development of the service. The evaluation shows that in several indicators the efficiency of the program in other countries, but the costs are very high and the payment set by the state does not correspond to the actual manipulations and the costs of the medical institution. The author offers to review the organization of the program, the writer has developed two alternative algorithms of the program, as a result of which recommendations have been developed for the implementation of a more appropriate and cost-effective approach to the needs of patients and the care process. The research has a scientific novelty and its results have practical application. The author offers a methodology for evaluating the costs and effectiveness of the program, and for the first time in Latvia, the results of the Decubitus treatment program were evaluated, costs were calculated and recommendations were developed for the further development of the program.
