Abstract

Uz pacientu centrētas primārās veselības aprūpes īstenošanas novērtējums Vidzemes reģionā. Pētījuma aktualitāte redzama jau nosaukumā – veselības aprūpe bija, ir un vienmēr būs aktuāls temats. Un primārā veselības aprūpe, kā tās pamats, ir viens no būtiskākajiem priekšnoteikumiem visas veselības aprūpes sistēmas veiksmīgai funkcionēšanai. Pētījuma galvenais mērķis ir izvērtēt to, kādā mērā ģimenes ārsti Vidzemes reģionā īsteno uz Pikera (kas ir pētījumam izvēlētais ietvars) pacientu centrētas veselības aprūpes principus, veicot savu pacientu aprūpi. Pētījuma galvenie uzdevumi ietver Nacionālā veselības dienesta publiskotās informācijas par ģimenes ārstu darbību apkopojumu, pacientu un ārstu/ speciālistu viedokļa apkopošanu un, izejot no iegūtiem rezultātiem, izstrādāt pamatotus ieteikumus primārās veselības aprūpes uzlabošanai. Divas no trīs izvirzītajām hipotēzēm pētījuma gaitā tika apstiprinātas. Pierādīta tika hipotēze, kas apgalvo, ka, ja uz pacientu centrētas veselības aprūpes principi tiek īstenoti zemākā mērā, pacienti šo ārstu vēlēsies rekomendēt mazāk. Visvājāk īstenotie principi, ņemot vērā pacientu viedokli ir pieejamība, ārstēšanas nepārtrauktība un iesaistīšana ārstēšanā, savukārt ārstu/ speciālistu viedoklis ir tāds, ka visvājāk īstenota ārstniecības kvalitāte. Viens no galvenajiem secinājumiem, kas jāņem vērā, veicot uzlabojumus vai mainot kaut ko veselības aprūpes sistēmas darbībā ir tāds, ka pacientu un ārstniecības personāla viedoklis par vienu un to pašu jautājumu atšķirsies, līdz ar to ir jāņem vērā visu iesaistīto pušu viedoklis. Evaluation of implementation of patient centred primary health care in Vidzeme region. The relevance of the research can be seen already in the title - health care was, is an always will be a relevant topic. And primary health care, which is the foundation of the healt care, is one of the most important prerequisites for the successful functioning of the entire health care system. The main goal of the study is to evaluate the extent to which family doctors in the Vidzeme region empower Picker's (which is the theoretical framework chosen for this study) principles of patient-centred health care. The main tasks of the study include the compilation of information published by National Health Service about family doctors, the collection of data from questionnaires of patients and structured interviews of doctors/specialists, and based on the obtained results, developing reasonable recommendations for improving primary health care. Two of the three hypotheses were confirmed. The hypothesis, which states that if the principles of patient-centred healthcare are implemented to a lower extent, patients will be less willing to recommend their doctor, was approved. The least implemented principles, are accessibility, continuity of treatment and involvement of the patient in treatment (according to the results of patient questionnaires), while the opinion of doctors/specialists differ and their opinion is that the quality of treatment is least implemented. One of the main conclusions that must be taken into account when making improvements or changing something in the health care system is that the opinion of patients and medical personnel will differ on the same issue, therefore the opinion of all parties involved must be taken into account.
