Ordināciju skrīninga algoritmu izstrāde un aprobācija farmakoterapijas problēmu identificēšanai internās medicīnas klīnikā.

Abstract

Līdz ar medicīnas un farmācijas straujo attīstību attīstās arī klīniskās farmācijas joma. Ir valstis, kur klīniskās farmācijas joma ir attīstījusies straujāk, kā piemēram, ASV vai Nīderlande Eiropā, tāpēc ir ļoti svarīgi mācīties no šīm valstīm un uzlabot jomu arī citās valstīs, tādā veidā uzlabojot pacienta aprūpi un dzīves kvalitāti. Maģistra darbs izstrādāts balstoties uz praksi esot Erasmus medical center, Roterdamā, Nīderlandē un iegūto materiālu – klīnisko protokolu jeb algoritmu aprobāciju Paula Stradiņa klīniskās universitātes slimnīcā, Latvijā. Darba mērķis ir apkopot informāciju par pieejamajiem protokoliem klīniskā farmaceita ikdienas darbā Erasmus klīniskās universitātes slimnīcā Nīderlandē un aprobēt tos Paula Stradiņa klīniskās universitātes slimnīcā endokrinoloģijas un reimatoloģijas nodaļā. Pētījumā tika konstatēts, ka klīniskie protokoli jeb algoritmi ir efektīvs palīglīdzeklis klīniskās farmācijas praksē kā ātri un efektīvi atrast farmakoterapeitiskās problēmas. Pētījums ir noritējis veiksmīgi. Pavisam tika apkopoti 181 gadījumi, atrastas 47 farmakoterapeitiskās problēmas, no kurām 14 reizēs veiktas intervences ar 100% pozitīvu apstiprinājumu no ārstējošo ārstu puses, kā arī ārsti ir ļoti pretimnākoši un izrāda vēlmi iesaistīties. Uzmanība jāvērš uz atrastajām augsta riska problēmām – dubultmedikācijas, mijiedarbības, kontrindikācijas, pārāk augstas vai zemas zāļu devas, kā arī zāļu ievades problēmas. The field of clinical pharmacy is also developing alongside the rapid evolution of medicine and pharmacy. There are countries where clinical pharmacy has developed faster, such as Amerika or the Netherlands in Europe and many others, so it is very important to learn from these countries and improve the field in other countries, thereby improving patient care and quality of life. The master thesis is based on the internship at Erasmus medical centre, Rotterdam, the Netherlands and the obtained material - clinical protocols or algorithm validation at Paula Stradins clinical university hospital, Latvia. The work aims to collect information about available protocols in the daily work of a clinical pharmacist at Erasmus clinical university hospital in the Netherlands and to validate them in the department of endocrinology and rheumatology at Paula Stradins clinical university hospital. The study found that clinical protocols or algorithms are an effective tool in clinical pharmacy practice for finding pharmacotherapeutic problems quickly and efficiently. The study has been successful. A total of 181 cases were collected and 47 pharmacotherapeutic problems were found, from which 14 cases of interventions were performed with 100% positive response rate from the treating physicians, and the physicians are very receptive and willing to be involved. Attention should be focused on the high-risk problems found - double medication, interactions, contraindications, over- or under-dosing, and problems with the administration of medicines.
