Šī darba mērķis bija novērtēt, kā akūta aeroba fiziskā slodze ietekmē miokīnu IL-6 un M-CSF koncentrāciju krūts vēža pacientēm, un vai šīs izmaiņas atšķiras pirms un pēc terapijas kursa. Pētījumā piedalījās 43 sievietes ar krūts vēzi, kurām asins paraugi tika ievākti gan pirms, gan pēc akūtas aerobas slodzes divos laika punktos – pirms terapijas un pēc tā. Imūnanalīzes tika veiktas ar Luminex® xMAP® daudzfaktoru analīzes tehnoloģiju. Pirms terapijas akūtas slodzes rezultātā IL-6 koncentrācija palielinājās no 3.25 pg/ml (IQR: 1.85–4.63) līdz 4.45 pg/ml (IQR: 2.90–5.74), (p = 0.015), bet pēc terapijas izmaiņas no 3.70 pg/ml (IQR: 2.78–5.66) līdz 3.73 pg/ml (IQR: 2.91–6.01) nebija statistiski nozīmīgas (p>0.05). Savukārt akūtas slodzes inducētas M-CSF koncentrācijas izmaiņas bija statistiski nozīmīgas pirms terapijas 182.90 pg/ml (IQR: 121.83–265.83) vs. 192.75 pg/ml (IQR: 129.93–282.45), bet pēc terapijas rādījumu samazinājās 205.20 pg/ml (IQR: 132.88–395.00) vs. 183.80 pg/ml (IQR: 111.73–355.13). Starp akūtas slodzes inducētām Δ IL-6 un Δ M-CSF koncentrācijām tika novērota statistiski ticama pozitīva korelācija (ρ = 0.644, p = 0.0129), kā arī spēcīga korelācija starp šo biomarķieru absolūtajām vērtībām visos laika punktos. IL-6 ir robusts slodzes inducēts miokīns, kas potenciāli var tikt izmantots kā biomarķieris personalizētās fiziskās rehabilitācijas uzraudzībā krūts vēža pacientēm. M-CSF gadījumā nepieciešami padziļināti pētījumi ar lielāku dalībnieku skaitu, lai precizētu tā lomu onkoloģiskās ārstēšanas kontekstā. Šī pētījuma novitāte ir tajā, ka tas ir viens no pirmajiem Latvijā, kur vienlaikus tiek vērtēta gan IL-6, gan M-CSF dinamika atbildes reakcijā uz slodzi, izmantojot reālas pacientu grupas pirms un pēc vēža terapijas, tādējādi sniedzot priekšstatu par iespējamiem bioloģiskajiem mehānismiem, kas varētu tikt iekļauti nākotnes multidisciplinārās ārstēšanas un rehabilitācijas pieejās.
The aim of this study was to evaluate how acute aerobic physical exercise affects the concentration of myokines IL-6 and M-CSF in breast cancer patients, and whether these changes differ before and after the course of therapy. The study involved 43 women with breast cancer, from whom blood samples were collected both before and after acute aerobic exercise at two time points – before therapy and after it. Immunoassays were performed using Luminex® xMAP® multiplex analysis technology. Before therapy, IL-6 concentration increased from 3.25 pg/ml (IQR: 1.85–4.63) to 4.45 pg/ml (IQR: 2.90–5.74), (p = 0.015), whereas after therapy, the change from 3.70 pg/ml (IQR: 2.78–5.66) to 3.73 pg/ml (IQR: 2.91–6.01) was not statistically significant (p > 0.05). M-CSF concentration before therapy increased statistically significant from 182.90 pg/ml (IQR: 121.83–265.83) to 192.75 pg/ml (IQR: 129.93–282.45), (p > 0.05), while after therapy it decreased from 205.20 pg/ml (IQR: 132.88–395.00) to 183.80 pg/ml (IQR: 111.73–355.13). As a result of acute exercise, a statistically significant positive correlation was observed between Δ IL-6 and Δ M-CSF (ρ = 0.644, p = 0.0129), as well as a strong correlation between the absolute values of these biomarkers at all time points. IL-6 is a reliable exercise-induced myokine whose changes may serve as a basis for future research into its use as a biomarker for monitoring personalized physical rehabilitation in breast cancer patients. In the case of M-CSF, more in-depth studies with larger participant numbers are required to clarify its role in the context of oncological treatment. The novelty of this study lies in the fact that it is one of the first in Latvia to simultaneously evaluate the dynamics of both IL-6 and M-CSF in response to exercise, using real patient groups before and after cancer therapy, thereby providing insight into possible biological mechanisms that could be included in future multidisciplinary treatment and rehabilitation approaches.
