Assessment of Pain Intensity by Nociception Level Index vs. Visual Analogue Scale during Epidural Analgesia at the Early Stages of a Delivery Process

Ievads: Epidurālā analgēzija, kas, kā zināms, bloķē simpātiskos stimulus, ir efektīva dzemdību sāpju mazināšanas metode. Sāpju intensitātes kontrolei ir izstrādāti vairāki objektīvi fizioloģiskie parametri, kurus kontrolē simpātiskā nervu sistēma. Viens no tiem ir nocicepcijas līmeņa (NOL) indekss. Pašreizējā pētījuma mērķis bija novērtēt NOL indeksu grūtniecēm dzemdību laikā pēc epidurālās blokādes ievadīšanas un atrast korelāciju ar pašu paziņoto vizuālo analogo skalu (VAS). Mērķi: Galvenie divi pētījuma mērķi ir novērtēt NOL indeksa vērtības dzemdējošo sieviešu vidū un izpētīt NOL indeksa vērtību korelāciju ar pašu uzrādīto VAS dzemdību sākumposmā, pirms un pēc epidurālās blokādes ievadīšanas. Metodes: Kopumā pētījumam tika piesaistītas 30 grūtnieces dzemdību sākumposmā, vecākas par astoņpadsmit gadiem pirms epidurālās atsāpināšanas. Tika iekļautas 19 grūtnieces. NOL indekss un VAS tika mērīti nepārtraukti, sākot no 2–3 dzemdes kontrakcijām pirms epidurālās analgēzijas ievadīšanas un ilga aptuveni 25–30 minūtes, līdz sāpes mazinājās katrā kontrakcijā. VAS rādītāji tika savākti tajā pašā laikā katras kontrakcijas beigās. Rezultāti: Augstāka korelācija starp VAS rādītājiem un NOL indeksu (40,9% pret 4,5%, P<0,01), starp NOL indeksu un dzemdes kontrakcijām (38,1% pret 4,8%, P<0,01); un starp VAS rādītājiem un kontrakcijām (atšķirība nebija nozīmīga), tika novērota augstāka korelācija pirms epidurālās blokādes ievadīšanas, salīdzinot ar zemāku korelāciju pēc epidurālās blokādes. Secinājums: Augstāka korelācija starp VAS rādītājiem un NOL indeksu tika novērota pirms epidurālās blokādes ievadīšanas, salīdzinot ar korelāciju pēc tās Tādējādi NOL indekss varētu kalpot kā objektīvs marķieris, lai novērtētu sāpju intensitāti dzemdību laikā, ļaujot veikt mērķtiecīgu epidurālo analgēziju.
Background: Epidural analgesia, known to block sympathetic stimulus, is an effective method of labor pain relief. Several objective physiological parameters, controlled by the sympathetic nervous system, have been developed to monitor pain intensity. One of those is the nociception level (NOL) index. The current study aimed to assess the NOL index in pregnant women during delivery after epidural block administration and to find correlation with the self-reported visual analog scale (VAS). Goals: The main two goals of the research are to evaluate the NOL index values among women in labor and to explore the correlation of the NOL index values to the self-reported VAS during the early stages of childbirth, before and after administration of epidural block. Methods: A total of 30 pregnant women at the early stages of the delivery, eighteen years and older, before epidural analgesia, were recruited for the study. 19 pregnant women were included. The NOL index and the VAS were measured continuously, starting from 2-3 uterine contractions before the epidural analgesia administration and lasted for approximately 25-30 minutes, till pain relief at each contraction. The VAS scores were collected during the same time, at the end of each contraction. Results: A higher correlation between the VAS scores and the NOL index (40.9% vs. 4.5%, P<0.01), between the NOL index and uterine contractions (38.1% vs. 4.8%, P<0.01); and between VAS scores and contractions (the difference was not significant), was observed in more cases before the epidural block administration compared to lower correlation after the epidural block. Conclusions: A higher correlation between the VAS scores and the NOL index was observed before epidural block administration compared to correlation after it. Thus, the NOL index could serve as an objective marker to assess pain intensity during labor, enabling target-guided epidural analgesia.
