Abstract
Darba nosaukums. Unilaterālu spēka treniņu efektivitāte pēc priekšējās krusteniskās saites
rekonstrukcijas: sistemātisks literatūras apskats.
Pamatojums. Neskatoties uz to, ka priekšējās krusteniskās saites rekonstrukcijas operāciju
tehnoloģijas arvien uzlabojas, rezultāti pēc operācijām joprojām ir vāji. Četrgalvainā muskuļa
vājums un asimetriska apakšējo ekstremitāšu slogošana ir divas galvenās problēmas pēc
priekšējās krusteniskās saites rekonstrukcijas. Unilaterāli treniņi papildus standarta procedūrām
ir uzlabojuši terapeitisko iznākumu pacientiem ar dažādām ortopēdiskām traumām. Unilaterāla
treniņa fenomens ir spēka uzlabojums netrenētajā ekstremitātē pēc spēka treniņa veikšanas
pretējās puses ekstremitātē.
Mērķis. Aprakstīt un analizēt unilaterālu spēka treniņu efektivitāti četrgalvainā muskuļa spēka
uzlabošanā rehabilitācijas posmā pēc priekšējās krusteniskās saites rekonstrukcijas.
Metodes. Publikāciju meklēšana veikta PubMed, PEDro, EBSCOhost, Embase pēc noteiktiem
atslēgas vārdiem: ACLR, ACL reconstruction, anterior cruciate ligament reconstruction, cross-training, contralateral, cross-exercise un unilateral training. Apskatā iekļauti randomizēti
kontrolēti pētījumi ar dažādu unilaterālu spēka treniņu programmu.
Rezultāti. Pētījuma daļā analizētas piecas publikācijas ar dažādām unilaterālu spēka treniņu
vingrojumu programmām. Pētījumos kopā tika iesaistīti 177 dalībnieki. Trijos pētījumos tika
novērota četrgalvainā muskuļa spēka uzlabošanās. Divos pētījumos netika sasniegts terapijas
metodes pozitīvs efekts.
Secinājumi. Submaksimālas intensitātes unilaterāls spēka treniņš kontralaterālās puses
četrgalvainajam muskulim personām pēc priekšējās krusteniskās saites rekonstrukcijas ir droša
un efektīva metode. Četrgalvainā muskuļa spēka uzlabošanos var panākt gan ar koncentrisku,
gan ekscentrisku muskuļu kontrakciju režīmu. Ekscentrisks muskuļu kontrakciju režīms ir
efektīvāks nekā koncentrisks muskuļu kontrakciju režīms.
Title. The Efficacy of Unilateral Strength Training after Anterior Cruciate Ligament Reconstruction: A Systematic Review of the Literature. Background. Despite the continuously increasing technology quality of anterior cruciate ligament reconstruction, the return to sports outcome is still low. Quadriceps muscle weakness and asymmetric gait are the two main issues after anterior cruciate ligament reconstruction. Cross education in addition to the usual rehabilitation protocols has shown therapeutic result improvement in patients with different orthopaedic injuries. The phenomenon of cross education is strength improvement in the operated limb after unilateral strength training of the contralateral non-injured limb. Objective. Describe and analyse current information on cross education effect on quadriceps muscle strength after anterior cruciate ligament reconstruction. Methods. Publication search was performed in online databases PubMed, PEDro, EBSCOhost, Embase by pre-selected keywords ACLR, ACL reconstruction, anterior cruciate ligament reconstruction, cross-training, contralateral, cross-exercise and unilateral training. The study includes randomized controlled trials with different programmes of cross education. Results. Five publications with different programmes of cross education were included in the study and a total of 177 participants were involved in this review. In three studies, clinically significant improvement of quadriceps strength was observed. In two studies no significant improvement was found. Conclusions. Submaximal intensity cross education in the contralateral non-operated limb is a safe and effective intervention that can improve the strength of the reconstructed limb post operatively. Quadriceps strength gains of the operated limb can be achieved by concentric or eccentric muscle contraction training where eccentric training shows more significant gains.
