Tikai kopš 2021. gada maija Latvijā ir ieviesta agrīna valsts apmaksātā priekšdziedzera vēža skrīningdiagnostika. Lai veicinātu skrīninga aptveri ir svarīga laba sadarbība starp ārstu, pacientu un valsts veselības aprūpes sistēmu, kas diemžēl bieži ir zemā līmenī.
Darba mērķis: Apzināt un analizēt vīriešu izpratni, zināšanas un atsaucību attiecībā uz agrīnu priekšdziedzera vēža skrīningdiagnostiku. Kā arī noskaidrot ģimenes ārstu atsaucību un iesaisti agrīnā priekšdziedzera vēža skrīningprogrammas ieviešanā.
Darba uzdevumi:
1. Veikt literatūras avotu analīzi par priekšdziedzera vēzi, tā skrīningdiagnostiku ģimenes ārsta praksē;
2. Izstrādāt un aprobēt anketas mērķpopulācijas (vīrieši vecumā no 45 – 75 gadiem) un ģimenes ārstu aptaujas veikšanai.
3. Ar elektroniskas anketēšana palīdzību, noskaidrot vīriešu viedokli (45-75gadi) par viņu zināšanām saistībā ar agrīnu priekšdziedzera vēža skrīninga diagnostiku Latvijā;
4. Ar elektroniskās anketēšanas palīdzību noskaidrot ģimenes ārstu viedokli saistībā ar mērķa populācijas atsaucību, zināšanām, skrīninga ieguvumiem un nepilnībām.
5. Iegūto datu apkopošana un analīze, secinājumu veikšana.
Darbā izmantotās metodes:
• Literatūras avotu analīze
• Vīriešu vecumā no 45 līdz 75 gadiem un Latvijas ģimenes ārstu anketēšana
• Datu apstrāde, izmantojot SPSS IBM Statistics (Hī kvadrāta tests, aprakstošas metodes)
Darba struktūra: Darbs sastāv no 43 lpp., kuras satur 13 attēlus, 4 tabulas, kā arī tam ir 2 pielikumi. Izmantoti 25 literatūras avoti.
Teorētiskajā daļā tika apkopota informācija par priekšdziedzera vēzi, tā epidemioloģiju, riska faktoriem, diagnostiku, ārstēšanu, klasifikāciju, prognozi, kā arī par skrīningprogrammu, tās priekšnosacījumiem un valsts apmaksāto priekšdziedzera vēža skrīningdiagnostiku Latvijā.
Pētnieciskajā daļā tika veikta vīriešu (45-75 gadi) elektroniska anketēšana, piedalījās 103 vīrieši, kas tika sadalīti divās pētījuma grupās (pirmā grupa-pilsētā dzīvojošie, bet otrā grupa lauku apvidos dzīvojošie). Kā arī ģimenes ārstu elektroniska anketēšana- piedalījās 77 ģimenes ārsti.
Pētījumā iegūtie secinājumi: Vīriešu zināšanas par valsts apmaksāto priekšdziedzera skrīninga diagnostiku (īpaši laukos un ciematos dzīvojošo) ir zemas, lielākā daļa par skrīningu nav informēti. Būtu nepieciešama papildus izglītošana un informēšana, lai veicinātu skrīninga aptveri un rezultātus.
Only since May 2021 population based early prostate cancer screening programme was introduced in Latvia. Good collaboration between doctors, patients, and the national health system is important to promote a screening responses rate, which is often at a low level. Research aim was to identify and analyse the understanding, knowledge, and responsiveness of men about early screening diagnostics for prostate cancer as well as to find out the responsiveness and involvement of family doctors in the introduction of an screening program. The main objectives of the research: 1. To carry out an analysis of the sources of literature on prostate cancer, its screening diagnostic in the practice of a family doctor 2. Developing and validating questionnaires for targeted populations (men aged 45-75) and family doctors. 3. To clarify the opinion of men (45-75 years) regarding their knowledge about early prostate cancer screening diagnostics in Latvia by electronic surveys 4. To clarify the opinion of family doctors on their views on the responsiveness of the target population, knowledge, screening benefits, and shortcomings by electronic surveys. The methods applied in the research • Analysis of sources of literature • Surveys of men aged 45 to 75 and family doctors in Latvia • Processing data using SPSS IBM Statistics v 27 (Chi-square test, descriptive methods) The research paper consist of: 43 pages, 4 tables, 13 pictures, 2 appendixes, used 25 information sources The theoretical part: information gathered on prostate cancer, epidemiology, risk factors, diagnostics, treatment, classification, forecast, as well as screening program, its prerequisites, and the early prostate cancer detection program The research part: an electronic survey of men (45-75 years) was carried out, involving 103 men divided into two study groups (the first group of people living in the city and the second group in rural areas). As well as an electronic survey of family doctors was carried out where 77 family doctors participated. The most important conclusion: Men's knowledge of early prostate cancer screening programme, (especially those living in fields and villages) is low, most of them are not informed about the screening. Additional education and information would be needed to promote screening response rate and outcomes.
