Anastomožu insuficience (AI) ir viena no bīstamākajām komplikācijām kolorektālajā ķirurģijā. Lai gan mūsdienās pastāv plašas anastomožu nepietiekamības diagnostikas un ārstēšanas iespējas, vienota stratēģija joprojām nav izstrādāta. Darba mērķis. Pētījuma mērķis bija noteikt iespējamos riska faktorus, kas var sekmēt kolorektālo anastomožu nepietiekamības attīstību, kā arī izpētīt anastomožu nepietiekamības diagnostikas un terapijas iespējas. Darba metodes. Retrospektīvā pētījumā tika iekļauti 65 pacienti, kuriem tika veikta resnās vai taisnās zarnas rezekcija ar primāras anastomozes izveidi Paula Stradiņa Klīniskajā universitātes slimnīcā laika posmā no 2020. gada janvāra līdz 2023. gada decembrim. Darbā tika analizēti pacientu demogrāfiskie, antropometriskie un laboratorie rādītāji pirms operācijas, kā arī operācijas tehniskie aspekti, pielietotās diagnostikas un ārstēšanas metodes. Datu apstrāde un statistiskā analīze veikta, izmantojot Microsoft Excel un IBM SPSS 29.0 datorprogrammas. Darba rezultāti. Pētījumā iekļauti 65 pacienti, no kuriem 33 (50,8 %) ir vīrieši un 32 (49,2 %) ir sievietes. Vidējais pacientu vecums ir 66,45 ± 13,27 gadi. No visiem pacientiem anastomozes nepietiekamība attīstījās 19 (29,2 %) gadījumos. Statistiski nozīmīga atšķirība tika noteikta starp neoadjuvantu staru un ķīmijterapiju un anastomozes nepietiekamības attīstību (p = 0,002). Visiem pacientiem AI grupā pirms operācijas tika veikta zarnu trakta sagatavošana ar laksatīviem līdzekļiem vai tīrošu klizmu un tika nozīmēta metronidazola un cefazolīna profilakse. Saistības starp anastomožu nepietiekamību un mehānisku zarnu trakta sagatavošanu, kā arī izolētu antibakteriālo profilaksi ar metronidazolu un cefazolīnu tika atzītas par statistiski būtiskām (p = 0,025 un p < 0,001). Laparoskopiskas pieejas izvēle un konvertācija no laparoskopiskas pieejas uz laparotomiju bija statistiski būtiski saistīta ar anastomožu nepietiekamību (p = 0,021 un p = 0,023). Četriem pacientiem diagnoze tika verificēta klīniski, bet pārējiem 15 pacientiem tika veikta DT vēderam un mazajam iegurnim ar vai bez intravenozu kontrastvielas ievadi. Visbiežāk sūdzības attīstījās 4. pēcoperācijas dienā un ietvēra sāpes vēderā (37,8 %), peritoneālos kairinājuma simptomus (10,8 %) un vēderizejas iztrūkumu (10,8 %). Radioloģiski pamatā tika konstatēts brīvs gaiss vai šķidrums vēdera dobumā (26,8 % un 19,5 %), kontrastvielas ekstravazācija (7,3 %) un ileusa aina (22 %). Konservatīva terapija ar plaša spektra antibiotikām bija iespējama vienā gadījumā. 10 (52,6 %) pacientiem anastomozi izdevās saglabāt, bet 8 (42,1 %) pacientiem bija nepieciešama anastomozes rezekcija. Secinājumi. Dotā atradne ļauj secināt, ka neoadjuvanta terapija, mehāniska zarnu trakta sagatavošana, antibakteriālā profilakse un operācijas pieeja var ietekmēt anastomožu nepietiekamības attīstības risku.
Anastomotic leakage (AL) is acknowledged as a significant complication in colorectal surgery. Although advancements have been made in the diagnosis and treatment methods of anastomotic leakage, an effective strategy has not yet been created. The objective of this study was to identify risk factors associated with anastomotic leakage and to evaluate diagnostic and treatment approaches for this complication. Methods. This is a retrospective study that was conducted at Pauls Stradiņš Clinical University Hospital and included 65 patients who underwent colon or rectal resection with primary anastomosis between January 2020 and December 2023. Patient demographics, physical measurements, preoperative laboratory results, surgical techniques, diagnostic methods, and treatment strategies were considered. Data processing and statistical analysis were performed using Microsoft Excel and IBM SPSS version 29.0 software. Results. The study included 65 patients in total, with 33 (50.8%) being men and 32 (49.2%) women. The mean age of the patients was 66.45 ± 13.27 years. Anastomotic leakage occurred in 19 (29.2%) of the patients. A statistically significant association was observed between neoadjuvant radiotherapy and chemotherapy and the occurrence of anastomotic leakage (p = 0.002). All individuals in the AL group underwent mechanical bowel preparation using either laxatives or a cleansing enema prior to surgery and were given prophylactic treatment with metronidazole and cefazolin. Conversely, 77.3% of patients in the non-AL group had mechanical bowel preparation, and 46.7% of them also received oral ciprofloxacin as part of their antibiotic prophylaxis. There were statistically significant associations between anastomotic insufficiency and mechanical bowel preparation, as well as the exclusive use of metronidazole and cefazolin (p = 0.025 and p < 0.001). The selection of a laparoscopic approach and the conversion from laparoscopy to laparotomy were both significantly related to anastomotic insufficiency (p = 0.021 and p = 0.023). Diagnosis was clinically confirmed in four patients, while the remaining 15 underwent abdominal and pelvic CT scanning, with or without intravenous contrast. The symptoms typically appeared on the 4th postoperative day, including abdominal pain (37.8%), signs of peritonitis (10.8%), and absence of bowel movement (10.8%). Imaging findings encompassed free air or fluid in the abdomen (26.8% and 19.5%), extravasation of contrast (7.3%), and ileus (22%). Conservative management with broad-spectrum antibiotics was possible in one instance. In 10 patients (52.6%), the anastomosis was salvaged, whereas 8 (42.1%) required a resection of the anastomosis. Conclusions. The results suggest that neoadjuvant therapy, mechanical bowel preparation, antibacterial prophylaxis, and the surgical approach could impact the likelihood of anastomotic leakage.
