Abstract
Ievads.
Introduction. Population tends to overweight and nonsurgical fat reduction and body contouring becomes popular solution to maintain well-being and improve health. The premise that the adipose tissue in the cellular formation is most susceptible to cooling when compared to other cells has promoted interest in the study it and other mechanisms to reduce localized adiposity. Aim. Investigate if the mechanisms of combined cryotherapy are effective on adipocytes. Methods. Systematical and analytical review of literature. Results. The best patient profile is minimally overweight, have noticeable fat bulges on the trunk, is realistic in their expectations, and is willing to maintain the results of cryolipolysis with a healthy, active lifestyle. Cryotherapy procedure appears to be effective and safe in the short term, with a limited side effect profile. Histologic findings supported the theory that cold exposure induce lipid crystallization and triggers selective adipocyte apoptosis without damage to surrounding tissues. The lipids released from apoptotic fat cells are slowly evacuated through the lymphatic system, thereby reducing fat tissue volume in the treated location, following by a macrophage-mediated process clears the cells over the proceeding 3 months. There is molecular sequence of apoptosis mediating cascade of enzymatic activation of proteins and number of important regulators in the extrinsic and intrinsic apoptotic pathway which leads to activation of substrate specific cysteinproteases. There are adipose specific genes that regulate adipocyte differentiation- nuclear factor (NFjB), peroxisome proliferators-activated receptor c (PPARc), cell death receptors CD95, TNFR1 and TNFR2. Conclusion. Reduction of adipocyte number by induced apoptosis by triggering regulator receptors may improve other therapeutic options as dietary supplements and creams, combined methods could be beneficial to use to reach better results of lipolysis. There is positive effect – reduction of adipocytes using combined cryotherapy methods to reach lipolysis and induce mechanism of apoptotic pathway into adipocytes.
