Šajā zinātniskās literatūras pārskatā apskatīta iespējamā saikne starp divām izplatītām mutes dobuma gļotādas slimībām, piemēram, atkārtotu aftozo stomatītu (RAS) un perorālo ķērpju plānu (OLP) un psiholoģiskajiem traucējumiem (trauksmi un depresiju). Tomēr RAS un OLP etioloģija joprojām nav zināma, jo vairākas hipotēzes atbalsta trauksmi un depresiju kā riska faktorus, kas varētu ietekmēt RAS un OLP attīstību un saasināšanos. Lielākā daļa zinātnisko rakstu ziņo par ievērojami augstāku stresa līmeni RAS un OLP pacientiem. Turklāt paaugstināts stresa biomarķieru līmenis, piemēram, kortizols siekalās un TNF-α, vēl vairāk apstiprina paaugstinātu stresa līmeni pacientiem. Un otrādi, pētījumi turklāt norāda, ka mutes dobuma gļotādas slimības var arī negatīvi ietekmēt psiholoģisko stāvokli un pazemināt dzīves kvalitāti. Tādējādi trauksme un depresija, kā arī mutes gļotādas slimības, piemēram, RAS un OLP, viena otru negatīvi ietekmē, parādoties komorbiditātē. Jāiesaka papildu un papildinošas ārstēšanas metodes zobārstam, kas apsver RAS un OLP psihosomatiskās izcelsmes iespēju.
This scientific literature review tackles the possible connection between two common oral mucosal diseases, such as recurrent aphthous stomatitis (RAS) and oral lichen planus (OLP) and psychological disorders (anxiety and depression). However, aetiology of RAS and OLP still remains unknown, multiple hypothesis advocate anxiety and depression being risk factors that might influence development and exacerbation of RAS and OLP. The majority of scientific articles report significantly higher levels of stress in RAS and OLP patients. Moreover, increased levels of stress biomarkers, such as cortisol in saliva and TNF-α further seem to confirm elevated stress levels in patients. Inversely, research moreover indicates that oral mucosal diseases might also negatively affect psychological state and decrease quality of life. Hence, anxiety and depression and oral mucosal diseases like RAS and OLP appear to influence each other negatively, emerging in comorbidity. Additional and complementing treatment methods for dentists must be suggested that consider possibility of psychosomatic origin of RAS and OLP.
