Ievads: Internalizējošie traucējumi ir vieni no biežāk sastopamajiem disstresa paušanas veidiem bērnu un pusaudžu populācijā. Šo simptomu gaita var būt visdažādākā – no fiziskām un emocionālām sūdzībām līdz problēmām attiecībās ar vienaudžiem. To nespecifiskās dabas dēļ, internalizējošie traucējumi bieži var palikt neatpazīti. Literatūrā ir maz datu par internalizējošo traucējumu prevelenci bērniem, īpaši tiem, kas jau saņem stacionāra aprūpi psihiatriskajā nodaļā.
Mērķis: Pētīt internalizējošo traucējumu izteiktību un to ietekmējošos faktorus bērniem un pusaudžiem, kas saņem stacionāra aprūpi bērnu psihiatrijas nodaļā.
Materiāli un metodes: Šajā šķērsgriezuma pētījumā ir iekļauti 59 bērni un pusaudži, kas saņēmuši stacionāra aprūpi Bērnu Klīniskās Universitātes slimnīcas Psihiatrijas nodaļā. Lai izvērtētu internalizējošo traucējumu prevalenci tika izmantota SDQ skrīninga instrumenta pašaptaujas (11-17 gadus veciem pusaudžiem) un vecāku aptaujas (2-10 gadu veciem bērniem) variants.
Rezultāti: Pētījumā tika iekļauti 59 dalībnieki – 33 (55,9%) meitenes un 26 (44,1%) zēni, ar vidējo vecumu 11,5 gadi (SD 4,72). Kopējā grūtību rezultāta 3 grupu kategorizācijā 24 (47,5%) dalībnieku rezultāti tika kategorizēti kā abnormāli. 28 (47,5%) dalībnieki uzrādīja borderline vai abnormālu emocionālu problēmu rezultātu un 36 (61%) dalībnieki uzrādīja borderline vai abnormālu rezultātu problēmās attiecībās ar vienaudžiem. Vidējais internalizācijas rezultāts bija 8,9 (SD 3,67) punkti no 20. Meitenes uzrādīja augstākus internalizācijas rezultātus nekā zēni (vidējais rezultāts 9,7 (SD 3,56) un 7,88 (SD 3,62)). Interesanti, ka meitenes uzrādīja augstākus emocionālo problēmu rezultātus, bet zēniem bija augstāki rezultāti problēmās attiecībās ar vienaudžiem.
Secinājumi: 4 no 10 bērniem, kas ārstējas bērnu psihiatrijas nodaļā, ir ievērojamas uzvedības un emocionālās grūtības. Katram trešajam bērnam, kas saņem psihiatrisko aprūpi, ir nopietnas emocionālās problēmas un problēmas attiecībās ar vienaudžiem. Līdz ar to internalizējošo traucējumu prevalence bērniem un pusaudžiem, kas saņem stacionāra aprūpi bērnu psihiatrijas nodaļā, ir augsta. Šīs augstās prevalences dēļ, ir ļoti svarīgi izprast un atpazīt internalizāciju, ļaujot samazināt riska faktorus un nodrošināt prevenciju.
Introduction: Internalizing symptoms are one of the most common ways to express distress in childhood and adolescence. The course of these symptoms can be various – from physical and emotional complaints to difficulties in relationships with peers. Due to nonspecific symptoms internalization can often be left unrecognized. There are limited data about the prevalence of internalizing symptoms in children, especially those who already are receiving inpatient psychiatric care. Objectives: To investigate the prevalence of internalizing symptoms in children and adolescents receiving inpatient psychiatric care. Materials and methods: This cross-sectional study includes 59 children and adolescents who received inpatient psychiatric care in Children’s Clinical University Hospital, Department of Psychiatry. To evaluate prevalence of internalizing symptoms Strengths and Difficulties Questionnaire’s (SDQ) self-report (adolescents aged 11-17) and parent report (children aged 2-10) forms were used. Results: 59 participants included 33 (55,9%) girls and 26 (44,1%) boys, with a mean age of 11,5 years (SD 4,72). In 3-band categorization of total difficulties score 24 (40,7%) individuals were categorized as abnormal. 28 (47,5%) participants showed borderline or abnormal emotional problems scores, and 36 (61%) showed borderline or abnormal peer problems scores. Mean internalization score was 8,9 (SD 3,67) out of 20. Girls showed higher internalization scores than boys (Mean 9,7 (SD 3,56) and 7,88 (SD 3,62) respectively). Interestingly, girls showed higher emotional problems scores, but boys had higher scores in peer problems. Conclusions: 4 out of 10 children in inpatient psychiatric care have significant emotional and behavioral difficulties. Every third child in psychiatric care faces serious emotional and peer problems. Consequently, prevalence of internalizing symptoms in children and adolescents receiving inpatient psychiatric care is high. Because of this high prevalence, it is extremely important to recognize and understand internalization, allowing to reduce risk factors and ensure prevention.
