Tiešas darbības perorālie antikoagulanti (TPOAK) ir izstrādāti tā, lai nodrošinātu efektīvu antikoagulāciju ar fiksētām devām, novēršot nepieciešamību pēc terapeitiskā zāļu monitoringa. Fiksētu devu shēmas plaši tiek izmantotas embolijas profilaksei pacientiem ar nevalvulāru ātriju fibrilāciju, pamatojoties uz klīnisko pētījumu datiem un izstrādātajām vadlīnijām. Tomēr pacientam specifisku riska faktoru analīze var sniegt papildu ieskatu precīzākai devas izvēlei dažādos klīniskajos scenārijos, īpaši tajos, kas var ietekmēt koagulācijas vai asiņošanas risku. Savlaicīga šo riska faktoru identificēšana var būtiski atvieglot ārstiem lēmumu pieņemšanu par antikoagulantu terapiju, tostarp par devas izvēli un augsta riska trombotisko vai asiņošanas notikumu novēršanu. Lai optimizētu terapiju un maksimāli uzlabotu pacientu iznākumu, ir būtiski izvēlēties atbilstošu TPOAK devu un ņemt vērā individuālos pacienta faktorus, lai samazinātu komplikāciju iespējamību. Šī pētījuma mērķis ir izstrādāt un pilotēt algoritmu TPOAK terapijas pārskatīšanai klīniskajā praksē, pamatojoties uz starptautiskajām vadlīnijām.. Šī algoritma ieviešana varētu uzlabot lēmumu pieņemšanu, uzlabot pacientu aprūpi un veicināt labāku antikoagulācijas terapijas pārvaldību.
Direct oral anticoagulants (DOACs) are designed to provide effective anticoagulation with fixed doses, eliminating the need for therapeutic drug monitoring. These fixed-dose regimens are widely used for embolism prevention in patients with non-valvular atrial fibrillation based on clinical trial data and established guidelines. However, analyzing patient-specific risk factors can offer additional insights for more precise dose selection in various clinical scenarios, especially those that may influence coagulation or bleeding risks. Timely identification of these risk factors can significantly aid clinicians in making informed decisions about anticoagulant therapy, including dose selection and preventing high-risk thrombotic or bleeding events. To optimize therapy and maximize patient outcomes, it is essential to select the appropriate DOAC dose and consider individual patient factors to minimize complications, adjusting the therapy as needed. This study aims to develop and pilot an algorithm for reviewing DOAC therapy in clinical practice, grounded in international guidelines. The implementation of this algorithm could improve decision-making, enhance patient care, and promote better management of anticoagulation therapy.
