Polimorbiditāte un polifarmācija kļūst par aizvien aktuālāku probēlēmu veselības aprūpes sistēmai. Ģimenes ārstiem ir galvenā loma primārajā aprūpē, tāpēc viņi bieži saskaras ar šādiem pacientiem.
Izmantojot klīniskā gadījuma izpēti, tiks aplūkotas šādu pacientu ārstēšanas grūtības, kā arī iespējamie risinājumi un stratēģijas no ģimenes ārsta perspektīvas. General practitioners play a central role in primary care and are therefore frequently confronted with such cases. Using a case study, the difficulties in treating such patients as well as possible solution strategies from the perspective of a GP will be discussed. Key points include the interconnected nature of heart-related diagnoses, the use of medications for hypertension, heart failure, and chronic kidney disease, and the management of atrial fibrillation with apixaban, bisoprolol, and digoxin. The discussion covers the impact of low glomerular filtration rate on platelet function and potential remedies. The second section addresses the patient's fall and second hospitalization, pointing out factors contributing to fall risk, including medications, muscle weakness, arrhythmias, and anaemia.
