Sarkoidoze ir multisistēmiska slimība, ko raksturo granulomu veidošanās, bieži skarot plaušas. Raksturīgās kalcija vielmaiņas izmaiņas potenciāli ietekmē slimības gaitu. Pētījuma mērķis bija izpētīt plaušu funkcionālo rādītāju (FVC%, DLCO%) un plaušu radioloģiskās ainas izmaiņas (RTG, CT) dinamikā pēc 1-2 gadiem un to iespējamu saistību ar kalcija vielmaiņu pacientiem ar pirmreizēji diagnosticētu sarkoidozi un hiperkalcēmiju Latvijā.
Plašā retrospektīvā pētījumā bija iegūti dati 816 pacientiem, kas ārstējušies Rīgas Austrumu Klīniskās universitātes slimnīcā no 2014. gada līdz 2018. gadam un kuriem bija pirmreizēji noteikta sarkoidozes diagnoze.
Kā pētījuma grupa tika izvēlēta pacientu apakšgrupa ar hiperkalcēmiju >2,55 mmol/l (n=65), izveidojot pēc vecuma un dzimuma saderinātu kontroles grupu ar pirmreizēji precizētu sarkoidozi un normālu seruma kalcija līmeni (n=75). No pieejamās dokumentācijas tika iegūti klīnisko, radioloģisko un plaušu funkcionālo izmeklējumu dati, veikta to analīze un salīdzināšana.
Radioloģisko izmaiņu progresiju noteica pēc sarkoidozes Scadding klasifikācijas pieauguma un atsevišķu radioloģisko elementu atradnes. Plaušu funkcionālo rādītāju progresija tika noteikta, pamatojoties uz FVC pazemināšanos par ≥10% un/vai DLCO pazemināšanos par ≥15% 1-2 gadu laikā pēc pirmreizējas diagnozes precizēšanas.
Radioloģisko izmaiņu pasliktināšanās bija izteiktāka hiperkalcēmijas grupā (26%), salīdzinot ar kontroles grupu (15%), bet plaušu funkcionālie rādītāji dinamikā biežāk pasliktinājās sievietēm, neatkarīgi no grupas.
54 no 65 pacientiem ar hiperkalcēmiju bija izvērtēts seruma kalcija līmenis dinamikā pēc 1-2 gadiem. Hiperkalcēmija persistēja 35% pacientu (n=19).
Nebija statistiski ticamas atšķirības starp grupām, vērtējot radioloģiskās ainas un plaušu funkcionālo rādītāju progresiju, neatkarīgi no dzimuma, lai gan apakšgrupu dati liecina par iespējamu saistību. Pacientiem ar sarkoidozi Latvijā ir daļēja saistība starp kalcija vielmaiņu un plaušu funkcionālajiem un radioloģiskās progresijas rādītājiem dinamikā.
Sarcoidosis is a multisystemic disease defined by the formation of granulomas in different organ systems. Changes in serum and 24 h urine calcium can influence the course of the disease. The goal of this study was to investigate the progression rate of pulmonary function (FVC%, DLCO%) and radiological changes (X-ray, CT), as well as the potential influence of calcium metabolism in patients with newly diagnosed sarcoidosis and hypercalcaemia. In a large retrospective study, medical data were collected from 816 patients who had been treated with a first-time confirmed diagnosis of sarcoidosis in the Riga East University Hospital between 2014 and 2018. From this data set with a first-time confirmed sarcoidosis, a group of 65 patients with hypercalcaemia >2.55 mmol/l for the current study was selected. An age- and gender-matched control group of 75 patients with normal serum calcium was formed. Clinical, radiological and pulmonary functional data were extracted and analysed from available patient documentation. Sarcoidosis progression was established based on deterioration of radiological Scadding stage changes or pulmonary function (decrease ≥10% of FVC and/or ≥15% DLCO) in one to two years after initial diagnosis. Radiological changes worsened more in the hypercalcaemia group (26%) compared with the control group (15%). Pulmonary function test values worsened more frequently in women over time, regardless of the study group. From 65 patients with hypercalcaemia, serum calcium was controlled in 54 cases between one to two years after initial diagnosis. Hypercalcaemia persisted in 35% of patients (n=19). There was no statistically significant difference between the groups in the progression rate of radiological changes and pulmonary function, regardless of gender, though subgroup data indicate a potential association. Patients with sarcoidosis in Latvia have a partial connection between calcium metabolism and the progression rate of radiological changes and pulmonary function changes.
