Apakšējo ekstremitāšu amputācijas ierobežo cilvēka spēju būt neatkarīgam, piedalīties sociālajā un darba dzīvē, kā arī ierobežo ikdienas aktivitātes un mobilitāti. Protēzes mērķis cilvēkam ir atgūt zaudēto mobilitāti un uzlabot dzīves kvalitāti. Tādējādi cilvēks vēlas būt apmierināts ar savu protēzi. Dzīves kvalitāti ietekmē ne tikai pacienta protēzes lietošana, bet arī saņemtā protezēšanas pakalpojuma kvalitāte. Latvijā pašlaik trūkst pētījumu, kas novērtē pacienta apmierinātību ar protēzi un protezēšanas pakalpojumu, un tā saistību ar dzīves kvalitāti.
Pētījuma mērķis: Novērtēt pacientu apmierinātību ar protezēšanas pakalpojumu un apakšējo ekstremitāšu protēzēm, un to saistību ar dzīves kvalitāti pieaugušajiem ar apakšējo ekstremitāšu amputācijām, veicot OPUS (Orthotics And Prosthetics Users Survey) aptauju trīs mēnešu periodā, lai noteiktu būtiskas korelācijas starp sniegto pakalpojumu, apmierinātību ar protēzi un dzīves kvalitātes rādītājiem.
Pētījuma dizains: Kvantitatīvs šķērsgriezuma pētījums.
Pētījuma metodes: Dati tika apkopoti izmantojot OPUS pašnovērtējuma aptauju, kas novērtē pacienta ekstremitāšu funkcionālo stāvokli, dzīves kvalitāti, apmierinātību ar protēzi un protezēšanas pakalpojumu. Pētījumā piedalījās 52 respondenti, kuriem ir veikta unilaterāla vai bilaterāla apakšējo ekstremitāšu amputācija transtibiālā vai transfemorālā līmenī, un pēdējo sešu mēnešu laikā lieto apakšējo ekstremitāšu protēzes. No pētījuma dalībniekiem papildus tika ievākti demogrāfiskie dati – vecums, dzimums, protēzes veids, protēzes lietošanas ilgums, amputācijas iemesls un veids. Respondenti tika aptaujāti telefoniski.
Rezultāti: Iegūtie dati par OPUS aptauju norāda, ka modulī "Apakšējo ekstremitāšu funkcionalitātes novērtējums (ApEFN)" vidējais punktu skaits ir 43 no 80. Modulī "Ar veselību saistītā dzīves kvalitāte (VDzK)" vidējais punktu skaits ir 60 no 92. Modulī "Klienta apmierinātība ar ierīci (AIe)" vidējais punktu skaits ir 43 no 55. Modulī "Klienta apmierinātība ar pakalpojumu (APa)" vidējais punktu skaits ir 46 no 50. Pīrsona korelācijas analīze tika veikta ApEFN, VDzK un AIe moduļiem, kur tika novērota vidēja, pozitīva korelācija (VDzK un ApEFN (r(50) = 0,524, p<0,001; VDzK un AIe (r(37) = 0,540, p<0,001). Spīrmena korelācijas analīze tika veikta, lai izpētītu saistību starp VDzK un APa moduļiem (starp VDzK un APa nepastāv korelācija un nav novērojama statistiski nozīmīga sakarība (p=0,056)).
Secinājumi: No aptaujas iegūtiem datiem ir secināms augsts pacientu apmierinātības līmenis saistībā ar protēzi un saņemto protezēšanas pakalpojumu. Tika iegūta vidēja korelācija starp pacientu apmierinātību ar protēzi un tās saistību ar dzīves kvalitāti, bet korelācija starp pacientu apmierinātību ar protezēšanas pakalpojumu un dzīves kvalitāti netika iegūta.
Lower limb amputations limit a person's ability to be independent, participate in social and work life, and also limits daily activities and mobility. Purpose of the prosthesis is to regain lost mobility and improve quality of life. Thus, a person wants to be satisfied with their prosthesis. Quality of life is affected not only by the use of a prosthesis, but also by the quality of the provided prosthetic service. Latvia currently lacks studies that assess patient satisfaction with prosthesis and prosthetic service, and its connection to quality of life. Aim: To assess patient satisfaction with prosthetic service and lower limb prostheses, and its connection with quality of life in adults with lower limb amputations by conducting the OPUS (Orthotics And Prosthetics Users Survey) self-report questionnaire over a three-month period to determine significant correlations between the provided service, satisfaction with the prosthesis and quality of life. Study design: Quantitative cross-sectional study. Research methods: Data was collected using the OPUS self-report questionnaire, which assesses the functional state of the patient's limbs, quality of life, satisfaction with the prosthesis and prosthetic service. The study included 52 respondents who have undergone unilateral or bilateral lower limb amputation at the transtibial or transfemoral level and had been using lower limb prostheses for the past six months. Additional demographic data was collected from the study respondents – age, gender, type of prosthesis, duration of prosthesis use, reason and the type of amputation. Respondents were interviewed by telephone. Results: The data obtained by the OPUS self-report questionnaire indicate that the average score in the module "Lower Extremity Functional Status (LEFS)" is 43 out of 80. The average score in the module "Health Related Quality of Life Index (HR-QOL)" is 60 out of 92. The average score in the module "Satisfaction with Devices (CSD)" is 43 out of 55. In the module "Satisfaction with Services (CSS)", the average score is 46 out of 50. Pearson correlation analysis was performed for the LEFS, HR-QOL and CSD modules, where a average, positive correlation was observed (HR-QOL and LEFS (r(50) = 0,524, p<0,001; HR-QOL and CSD (r(37) = 0,540, p<0,001). Spearman correlation analysis was performed to find the connection between the HR-QOL and CSS modules (there is no correlation between HR-QOL and CSS and no statistically significant relationship is observed (p=0,056)). Conclusions: The data from the survey indicates a high level of patient satisfaction with the prosthesis and the prosthetic service. There was an average correlation between patients' satisfaction with the prosthesis and its connection to quality of life, but no significant correlations between patients' satisfaction with the prosthetic service and quality of life.
