Kopsavilkums

Ievads: Autoimūns tiroidīts (AITD) ir autoimūna slimība ar visaugstāko izplatību baltās rases sabiedrībā. Tā kā autoimūnā tireoidīta izplatība pieaug, kamēr ģenētiskā predispozīcija saglabājas tāda pati, jāvērtē vides etiopatoģenēze. Mērķis: Vides faktoru un hroniska autoimūna tireoidīta saistības izprašana un identificēšana. Šis darbs ir vērsts uz konkrētu riska faktoru apzināšanos, kontroli un samazināšanu, lai novērstu iespējamo kaitējumu nākotnē. Materiāls un metodes: Literatūras apskates sagatavošanai tika izmantoti zinātniskie raksti, žurnāli, grāmatas un rekomendācijas. Literatūras avoti tika atlasīti no datu bāzēm Elsevier, PubMed, McGraw-Hill un Springer. Uzmanība tika pievērsta tam, lai vienā tēmā iekļautu tikai jaunākos empīriskos pētījumus. Rezultāt: Pārlieku liels joda daudzums organismā var izraisīt autoimūno tireoidītu. Selēnu nevar identificēt kā riska faktoru, bet papildterapija ar to sekmē autoantivielu samazināšanos un labsajūtas uzlabošanos. Jonizējošo starojumu varētu noteikt kā skaidru AITD riska faktoru. Dzīvsudrabu varētu uzskatīt par vairogdziedzera darbības traucējumu izraisītāju, bet līdz šim nav pierādīts, ka tas izraisa AITD cilvēkiem. Pētījuma rezultāti par PBDE ir ļoti atšķirīgi. Tāpēc to nevarēja skaidri identificēt kā riska faktoru cilvēkiem. Smēķēšanu varētu noteikt par Greivsa slimības riska faktoru. Turpretī, citos pētījumos tika pierādīts, ka smēķētāji mazāk pakļauti Hašimoto slimības attīstībai. Secinājumi: Klimata pārmaiņas un izmaiņas uzvedībā (diēta, elektroniskās nikotīna piegādes sistēmas) nākotnē radīs vēl nebijušu iedarbību, tāpēc lielu uzmanību jāpievērš cilvēku veselībai. Ieteicams pārbaudīt indivīdus, kas ir pakļauti AITD riska faktoriem. Ieteicams īstenot plašāku sabiedrības izglītošanu par riska faktoru iedarbību. Ir ieteicams ieviest sankcijas par riska faktoru izplatīšanu (ķīmisko vielu nejaušu noplūdi no uzņēmumiem, smēķēšanu). Lai noteiktu riska faktorus un to sinerģisko ietekmi uz uzņēmīgiem indivīdiem, jāveic vairāk pētījumu.
Introduction: Autoimmune Thyroiditis (AITD) is the autoimmune disease with the highest prevalence in the Caucasian society. As the prevalence of Autoimmune thyroiditis is rising while genetic predisposition keeps the same, the environmental etiopathogenesis must be accountable. Yet the connection of genetic influence is well better understood than the influence of exogenous influence. Aim: Understanding and identifying the association of environmental agents and chronic autoimmune thyroiditis. This thesis should contribute to the awareness, control, and reduction of specific risk factors in order to prevent possible harm in the future. Material and Methods: For the literature review scientific journals, articles, books and recommendations were used. They were selected from databases Elsevier, PubMed, McGraw-Hill and Springer. Attention was paid to include only the most recent empiric studies Results: Iodine in excess can lead to autoimmune thyroiditis. Selenium could not be identified as a risk factor, but its supplementation could show a reduction of autoantibodies and an improvement of the wellbeing. Ionizing radiation could be determined as a clear risk factor for AITD. Mercury could be identified as disruptor of the thyroid function, but so far not be proven to trigger AITD in humans. The study results about PBDEs are very mixed. Therefore, it could not be clearly identified as a risk factor for humans. Smoking could be identified as a risk factor for Grave’s disease. On the contrary studies could also prove that smokers are less affected by Hashimoto’s disease. Conclusions: The climate change and changes in behavior (diet, Electronic Nicotine Delivery Systems) will lead to yet unknow exposure in the future and therefore needs to be paid more attention to human health. Screening of individuals that are occupationally exposed to AITD risk factors is recommended. More education of society about the exposure to risk factors is advised to be implemented. Sanctions for the release of risk factors to the nature are recommended to be introduced (accidental release of chemicals by companies, smoking). In order to identify risk factors and their synergistic effects with susceptible individuals, more research needs to be done.
