Pētījuma mērķis Šī pārskata mērķis ir pievērsties tehniskajām problēmām un izpētīt reģionālās anestēzijas sekmīguma rādītājus aptaukojušiem pacientiem, koncentrējoties uz neiraksiālo anestēziju un perifēro nervu blokādēm. Pētījuma jautājums Pētījumā tiek noskaidrots, vai, veicot reģionālo anestēziju, aptaukojušos pacientus raksturo atšķirīgi veiksmes rādītāji salīdzinājumā ar pacientiem bez aptaukošanās. Pārskata mērķi 1. Apspriest galvenās tehniskās problēmas, veicot reģionālo anestēziju aptaukojušiem pacientiem. 2. Salīdzināt dažādas reģionālās anestēzijas metodes un analizēt ar tām saistītos panākumu rādītājus aptaukošanās gadījumā. 3. Novērtēt faktorus, kas saistīti ar reģionālās anestēzijas panākumiem pacientiem ar aptaukošanos, salīdzinot ar pacientiem bez aptaukošanās. 4. Izpētīt ultraskaņas vadīšanas un nervu stimulācijas tehnikas izmantošanu, lai uzlabotu reģionālās anestēzijas rezultātus. Metodes Šajā literatūras apskatā izmantoti pierādījumu bāzes dati no vairākām zinātniskajām datubāzēm, piemēram, UpToDate, BMJ, Elsevier, PubMed, European Journal of Anaesthesia. Secinājumi 1. Reģionālā anestēzija aptaukojušos pacientos, salīdzinot ar pacientiem, kas nav aptaukojušies, ir līdzīgi veiksmīga, un reti ir gadījumi, kad anestēzija ir pilnīgi neveiksmīga un ir jāpāriet uz citām anestēzijas metodēm. 2. Kopumā aptaukošanās palielina šīs prakses grūtības, parasti ietekmējot veikšanas laiku un anestēzijas sasniegšanu. 3. Ultraskaņas vadīšana, dažkārt ar papildu nervu stimulāciju, var uzlabot reģionālās anestēzijas rezultātus aptaukojušiem pacientiem.
Aim of the study This review intends to address the technical challenges and study the rates of success of regional anaesthesia in obese patients, focusing on neuraxial anaesthesia and peripheral nerve blocks. Research question The study inquiry whether the obese population experience different rates of success compared to non-obese when undergoing regional anaesthesia. Objectives of the Review 1. Discuss the main technical challenges of performing regional anaesthesia on obese patients 2. Compare different techniques of regional anaesthesia and analyse the associated rates of success in obese subjects 3. Evaluate the factors related to the success of regional anaesthesia in obese patient compared to non-obese 4. Study the use of ultrasound guidance and nerve stimulation technique to improve the outcomes of regional anaesthesia Methods This literature review refers to evidence-base data form several scientific databases such as UpToDate, BMJ, Elsevier, PubMed, European Journal of Anaesthesia. Conclusions 1. Regional anesthesia in the obese population, compared to non-obese, has similar success rates with rare instances of complete failure and need for conversion to other anesthesia methods. 2. Overall obesity increases the difficulty of this practice usually affecting the time of performance and achievement of anesthesia. 3. Ultrasound guidance, with additional nerve stimulation at times, can improve the outcomes of regional anesthesia in obese patients.
