Jūras eža granuloma ir hroniska granulomatoza ādas slimība, ko izraisa pastāvīgs un penetrējošs tā dzelonis, kas nav pilnībā izņemts no brūces. Jūras eži parasti ir dzeloņaini, lodveida dzīvnieki, Echinoidea klases adatādaiņi. Apmēram 940 sugas dzīvo jūras gultnē, apdzīvojot visus okeānus un dziļuma zonas. Aptuveni 80 to sugas ir toksiskas cilvēkiem. Visbiežāk tiek ievainotas pēdas, potītes un rokas. Ir aprakstītas dažādas izpausmes, kas klasificētas kā tūlītēji lokāli iekaisumi vai aizkavētas reakcijas. Aizkavētās reakcijas sastāv no mezglveida granulomatoziem bojājumiem.
Klīniskā gadījuma aprakstā tiek apskatīts 40 gadus vecs vīrietis, kurš dzīvo Roskildē, Dānijā. Pacients anamnēzē min, ka pēc niršanas viņš kreisajā kājā sajutis asas, durošas sāpes pēc saskarsmes ar jūras ezi. Pēc tam esot parādījusies eritēma, tūska, asiņošana un lokāla miaļģija. Pēc šī incidenta pacients nav vērsies pēc medicīniskas palīdzības. Tajā brīdī vīrietis bija pārliecināts, ka jūras eža dzeloni no brūces ir izvilcis pats ar saviem spēkiem. Gadu pēc traumas pacients tomēr vērsās pēc medicīniskas palīdzības sāpīga mezglveida veidojuma un tūskas dēļ kreisās pēdas mugurpusē.
Dermatoloģiskā izmeklēšana atklāja tūsku, hiperkeratozi un kompaktu, nenoteiktas izcelsmes purpursarkanu mezglu, kura diametrs bija aptuveni 0,7cm. Bojājums bija sāpīgs veicot manipulāciju. Tika veikta mezgla ekscīzija un biopsija. Pēc histopatoloģiskās izmeklēšanas rezultātiem ekscidētais bojājums sastāvēja no ne-kazeozām kompaktām šūnu kolekcijām ar epitēlija šūnām, Langhansa milzu šūnām un limfocītiem. Pēc jūras eža granulomas ekscīzijas cita specifiska terapija nebija nepieciešama.
Dermatologiem būtu jāpievērš uzmanība šai tēmai, jo praktiski visos indīgo ūdensdzīvnieku nodarītajos ievainojumos āda ir bojāta. Jūras eža dzeloņi var izraisīt tūlītējus simptomus, taču jūras eža granulomu var izprovocēt nepilnīgi no brūces izvilkts tā dzelonis. Galvenā ārstēšanas metode ir šī bojājuma (granulomas) ekscīzija.
Sea-urchin granuloma is a chronic granulomatous skin disease caused by frequent and successive penetration of sea urchin spines which have not been removed from wounds. Sea urchins are typically spiny, globular animals, echinoderms in the class Echinoidea. About 940 species live on the seabed, inhabiting all oceans and depth zones. Approximately 80 species of them are toxic to humans. The most commonly compromised areas are the feet, ankles and hands. A variety of manifestations have been described, classified as immediate-local inflammation or delayed reactions. The delayed types consist of nodular granulomatous lesions. A 40-year-old male living in Roskilde, Denmark. The patient mentioned that after the diving he felt a sharp, stabbing pain from the sea-urchin in his left foot. Afterwards appeared erythema, edema, bleeding and local myalgia. He did not look for medical assistance after incident. At that moment the man thought that he extracted the spine from the wound himself completely. Although he sought medical attention one year after the injury due to painful nodule and edema in the back of his left foot. Dermatological examination revealed edema, hyperkeratosis and compact, nondefined origin purple nodule around 0,7 cm diameter. The lesion was painful to manipulation. Further, the nodule excision and biopsy were performed. Histopathological exam demonstrated non-caseating compact collection of cells, with epithelioid cells, Langhans giant cells and lymphocytes. After sea-urchin granuloma removal, no other specific therapy was required. Dermatologists must pay attention to this subject as the skin is compromised in practically all injuries caused by poisonous aquatic animals. Sea-urchin spines can cause immediate symptoms. Though sea-urchin granuloma can be provoked by incompletely removed spines from wounds. The main treatment is excision of the lesion.
