The effect of perinatal phthalate exposure on neurodevelopment in children

Ievads: Ftalāti ir endokrīno sistēmu graujošas ķīmiskas vielas, kas ir atrodamas ikdienas produktu materiālos, tās viegli uzsūcas cilvēka organismā un metabolizējas. Pamatojoties uz atklājumiem, kas iegūti pētījumos ar dzīvniekiem, ir aizdomas, ka ftalāti var traucēt neironu attīstību, īpaši, ja bērni ir pakļauti lielam daudzumam organisma attīstības periodā. Iedarbība notiek jau dzemdē caur placentu un pēcdzemdību laikā ar mātes pienu un vides iedarbību (orālā ceļā ar pārtiku vai kontaktā ar produktu). Ftalātu izmantošanas ierobežojumi patēriņa precēs nav starptautiski reglamentēti, un pastāv šaubas par pieļaujamo līmeņu pietiekamību. Daudzos pētījumos, kas tika veikti par šo tēmu pēdējo divdesmit gadu laikā, netika atklāta skaidra saikne starp ftalātu iedarbību un neiroloģisku attīstību. Mērķis: Šī raksta mērķi ir: 1) sniegt pārskatu par līdzšinējo pētījumu statusu attiecībā uz ftalātu iedarbības un neiroloģiskās attīstības cēloņsakarību; 2) apskatīt izvēlēto publikāciju no 2020. līdz 2024. gadam. Darba uzdevumi: 1) izpētīt, vai ir novērstas iepriekšējo publikāciju nepilnības, kā arī kas ir atklāts jauns un kas jāuzlabo turpmākajos pētījumos; 2) jaunāko pētījumu rezultātu izklāsts attiecībā uz izvēlētajiem neiroloģiskās attīstības traucējumiem; 3) aprakstīt endokrīnās sistēmas traucējumu mehānismus un pamatinformāciju. Metodes: datu bāzēs PubMed un PRIMO tika meklēti, izmantojot atslēgvārdus “phthalates” un “neirodevelopment”, tostarp publikācijas no 2020. līdz 2024. gadam. No tiem 9 pētījumi tika atlasīti detalizētai pārbaudei, pamatojoties uz iepriekš noteiktiem kritērijiem. Turklāt 3 lieli literatūras apskati tika izmantoti kā vadlīnijas par iepriekšējo pētījumu stāvokli un nākotnes studiju plānošanas prasībām. Pamatinformācijai un tēmas izpratnei tika izmantotas papildu zinātniskās publikācijas. Rezultāti un secinājumi: Apskatītajos pētījumos ir novērsti vairāki iepriekšējo pētījumu trūkumi. Pētījumos ar dzīvniekiem ir atklāti pierādījumi par saistību starp ftalātu iedarbību un traucētu neironu attīstību, un ir atklāti to mehānismi. Kopumā rezultāti ir pretrunīgi, un nav skaidras saiknes starp ftalātu iedarbību un neironu attīstību bērniem.
Background: Phthalates are endocrine disrupting chemicals that are ubiquitous in products of everyday use and are easily absorbed and metabolized by the human body. Based on findings from animal studies they are suspected of impairing neurodevelopment, especially if children are exposed to high amounts during vulnerable phases. Exposure occurs already in the uterus via the placenta and postnatally through breastmilk and environmental exposure. Limitations on phthalate use in consumer products is not internationally regulated and there are doubts about the sufficiency of allowed levels. The numerous studies that have been conducted on this topic over the past twenty years have found limited evidence on the association between phthalate exposure and neurodevelopment. Additional data is needed to establish a clear association. Aim: The aim of this article is to give an overview over the previous research status regarding the causal relationship of phthalate exposure and neurodevelopment and examine selected publication from 2020 to 2024. It shall be investigated whether shortcomings of previous publications have been addressed and what remains to be improved in future research. The results of the latest studies shall be laid out in regards to chosen neurodevelopmental endpoints. Mechanisms of endocrine disruption as well as background information shall be presented. Methods: The databases PubMed and Primo were searched using the keywords phthalates and neurodevelopment including publications from 2020-2024. Of these, 9 studies were selected to be examined in detail based on previously established criteria. In addition, 3 large literature reviews were used as a guideline on the previous state of research and requirements of future study design. Additional scientific publication were used for background information and understanding of the topic. Results and Conclusions: Several of the shortcomings of previous research have been addressed in the examined studies. Animal studies have found evidence for the association between phthalate exposure and impaired neurodevelopment and mechanisms have been revealed. Overall, the results are contradictory and there is no clear pattern of association between phthalate exposure and neurodevelopment in children.
