Mērķis un uzdevumi: apkopot un analizēt zinātniskā literatūrā esošus pierādījumus par telerehabilitācijas metodes pielietošanas ergoterapijas praksē efektivitāti, sniedzot neirorehabilitācijas pakalpojumu personām pēc pārciesta insulta.
Dizains: sistemātisks literatūras pārskats.
Metodes: zinātniskās literatūras meklēšana, izmantojot sešas tiešsaistes datubāzes – ClinicalKey, EBSCOhost, ProQuest, PubMed, SAGE journals, ScienceDirect; atrasto pētījumu kritiska analīze un novērtēšana, izvēle pēc atbilstības atlases kritērijiem; aprakstošā iegūto datu analīze, rezultātu apkopošana.
Rezultāti: kopā tika atrasti 3837 literatūras avoti no sešām tiešsaistes datubāzēm. Vairākas reizes veicot atlasi, literatūras pārskatā tika iekļauti un analizēti divi pētījumi, kuri atbilda visiem iekļaušanas kritērijiem – tie tika veikti 2012. un 2015.gadā, pēc dizaina ir randomizēti kontrolēti pētījumi. Abu pētījumu metodoloģiskā kvalitāte tika novērtēta kā augsta. Kopā pētījumos piedalījās 189 dalībnieki, kuru vidējais vecums ir 57 gadi un kuri pārcieta insultu. Katra pētījuma dalībnieki nejaušā kārtā tika sadalīti divās grupās – eksperimenta un kontroles. Pētījumu ietvaros dalībnieki, ar viņu aprūpētāju aktīvu līdzdalību, pielietoja dažāda veida rehabilitācijas tehnoloģijas, atrodoties mājas vidē. Tehnoloģijas iekļāva ar videodisku vadītu terapiju, mājas vingrinājumu programmas un robottehnoloģiju skartās augšējās ekstremitātes funkciju uzlabošanai. Šo tehnoloģiju efektivitāte un ietekme uz dalībnieku neatkarības līmeni, dzīves kvalitāti un depresijas simptomiem tika novērtēta pētījumu ietvaros, izmantojot standartizētus novērtēšanas instrumentus. Gan eksperimenta, gan kontroles grupu dalībniekiem intervences beigās tika novēroti uzlabojumi, taču netika novērota izteiktā rezultātu dažādība starp dažādu intervenču grupām.
Secinājumi: randomizēto kontrolēto pētījumu daudzums ir nepietiekams, lai novērtēt telerehabilitācijas metodes pielietošanas ergoterapijas praksē efektivitāti, sniedzot neirorehabilitācijas pakalpojumu personām pēc pārciesta insulta. Balstoties uz analizēto pētījumu rezultātiem var pieļaut, ka telerehabilitācijas metode var kalpot par vērtīgu rīku, sniedzot neirorehabilitācijas pakalpojumus.
Title of study: Effectiveness of the use of telerehabilitation method in occupational therapy practice providing neurorehabilitation services to persons after stroke. Research problem: Is the use of telerehabilitation method efficient in the occupational therapy practice providing neurorehabilitation services to individuals who had a stroke? Aim and tasks: to compile and analyze evidences that address efficiency of the use of telerehabilitation providing neurorehabilitation services to individuals who had a stroke. Design: systematic literature review. Methods: searching for scientific literature via six online databases – ClinicalKey, EBSCOhost, ProQuest, PubMed, SAGE journals, ScienceDirect; critical analysis and evaluation of founded researches, research selection according to the eligibility criteria; descriptive analysis of the obtained data, compilation of results. Results: a total of 3837 literature sources were found form six online databases. After several selections the literature review included and analyzed two studies that met all inclusion criteria – they were conducted in 2012 and 2015 and the design was randomized controlled trial. The methodological quality of both studies was assessed as high. A total of 189 participants with a mean age of 57 who had stroke participated in the studies. Participants in each study were randomly divided into two groups – experimental and control. As part of the research, participants, with the active participation of their caregivers, used different types of rehabilitation technologies while being in home environment. Technologies included video-disc guided therapy, home exercise programs and robotic device that is used to improve affected upper extremity functions. Efficiency and impact of these technologies on participants’ level of independency, quality of life and depressive symptoms were assessed in the studies using standardized assessment tools. Improvements were observed in both – experimental and control groups at the end of the intervention, but no marked variability in results was observed between different intervention groups. Conclusions: the number of randomized controlled trials is insufficient to assess the efficiency of the use of telerehabilitation method in occupational therapy practices providing neurorehabilitation services to individuals who had a stroke. Based on the results of the analyzed researches it can be assumed that telerehabilitation method can serve as a valuable tool in providing neurorehabilitation services.
