Priekšvēsture: Diabētiskā makulas tūska ir visizplatītākais redzes zuduma cēlonis pacientiem ar diabētisko retinopātiju. Anti-VEGF zāļu izstrāde ieviesa jaunu DME ārstēšanas laikmetu. Pēdējos gados tirgū parādījās jaunizveidoti anti-VEGF līdzekļi. Mērķis: Šī darba mērķis ir pārskatīt pieejamo angļu literatūru par dažādiem anti-VEFF līdzekļiem un iepazīstināt ar to efektivitāti un drošību diabētiskās makulas tūskas ārstēšanā. Metodes: Meklēšana PubMed datubāzē, izmantojot atslēgvārdus “Diabētiskā makulas tūska”, “DME”, “anti-VEGF”, “Pegatinib”, “Ranibizumab”, “Bevacizumab”, “Aflibercept”, “Faricimab”, “Refractory DME” ar mērķis ir atrast pārskata rakstus un primārās publicētās klīniskās takas. Secinājumi: Katrs anti-VEGF līdzeklis klīniskajos pētījumos uzrādīja nozīmīgu funkcionālo un anatomisko rezultātu uzlabošanos. Lēmumam par to, kuru anti-VEGF izvēlēties, pamatojoties uz DRCR.net protokola T (2015) rezultātiem, vajadzētu būt atkarīgam no pacienta sākotnējā/sākotnējā vislabāk koriģētā redzes asuma. Neskatoties uz revolucionārajiem anti-VEGF līdzekļu ārstēšanas panākumiem pacientiem ar DME, pacientu ar ugunsizturīgu DME procentuālais daudzums joprojām ir augsts. Šie ugunsizturīgie pacienti ir terapeitisks izaicinājums, daži no viņiem var gūt labumu no pārejas uz augstas afinitātes anti-VEGF zālēm, piemēram, Aflibercept.
Background: Diabetic macular edema is the most common cause of vision loss in patients with diabetic retinopathy. The development of anti-VEGF drugs introduced a new era of DME treatment. In recent years, newly developed anti-VEGF agents emerged on the market. Objective: This work aims to review the available English literature on the different anti-VEFF agents and present their efficacy and safety in the treatment of diabetic macula edema. Methods: A search of the PubMed database using the keywords “Diabetic macula edema”, “DME”, “anti-VEGF”, "Pegatinib", "Ranibizumab", "Bevacizumab", "Aflibercept", "Faricimab", "Refractory DME" to find review articles and primary published clinical trials. Conclusion: Every single anti-VEGF agent demonstrated significant improvement in functional and anatomical outcomes in clinical trials. The decision of which anti-VEGF to choose, based on results from the DRCR.net Protocol T (2015), should depend on the initial/baseline best corrected visual acuity of the patient. Despite the revolutionary treatment success of anti-VEGF agents in patients with DME, the percentage of patients with refractory DME remains high. Those refractory patients represent a therapeutic challenge, some of them may benefit from switching to a high-affinity anti-VEGF drug like Aflibercept.
