Abstract
Ievads: Garīgais un fiziskais izsīkums tiek uzskatīts par arodbīstamību, kā arī plaši izplatītu parādību, kas ietekmē medicīniskā personāla darba spējas, nodrošināto veselību un pacientu drošību. Izsīkuma sekas var būt nedrošas, izraisot medicīniskās kļūdas, pasliktinātu medicīnisko vērtējumu un samazinātu efektivitāti darbā.
Mērķis: Šī darba mērķis ir analizēt garīgās un fiziskās noguruma negatīvo ietekmi uz veselības aprūpes darbiniekiem un viņu pacientu aprūpi, izpētīt iespējamos riska faktorus, kā arī preventīvos faktorus.
Metodes: Informācijas apkopošanai, lai nobeigtu šo tēmu, ir meklētas tiešsaistes datubāzes, piemēram, PubMed, Elsevier, Clinic Key, Google scholar, ScienceDirect u.c. Tika izvēlēta un novērota labāko rakstu atlase, kur prezentētas galvenās idejas un rezultāti. šajā dokumentā.
Rezultāti: Izvērtējot šos daudzos rakstus par spēku izsīkumu un tā nelabvēlīgo ietekmi uz pacientu aprūpi, tie norādīja uz pozitīvu saistību starp garīgo un fizisko izsīkumu un pieļauto medicīnisko kļūdu palielināšanos, kas negatīvi ietekmēja pacientu aprūpi. Tika arī norādīts, ka medicīnas darbiniekiem, kuriem ir depresija, ir lielāks medicīnisko kļūdu risks. Nogurumu veicinoši riska faktori tika konstatēti šādi: palielināta darba slodze un darba laiks, organizatorisko faktoru traucējumi, psihosociālā darba vide, kā arī ilggadēja pieredze, kā arī jaunajiem ārstiem - paaugstināts medicīnisko kļūdu risks. Tika novērtēti arī preventīvie faktori, kas uzrādīja pozitīvu efektu, savlaicīgi iejaucoties un tos novēršot. Ir svarīgi ņemt vērā un pilnveidot organizatoriskos faktorus, regulēt darbinieku darba laiku un slodzi, kā arī ieguldīt tādos individuālos faktoros kā: stresa menedžmenta apmācība, uzmanīgums, pašaprūpe un komunikācijas prasmes. Ir pierādīts, ka šie faktori palīdz mazināt ārstu un medmāsu nogurumu.
Secinājums: Noslēgumā jāsaka, ka no vienpadsmit novērtētajiem rakstiem, septiņi norādīja uz pozitīvu saistību, tāpēc palielināts spēku izsīkums ir ļoti saistīts ar nelabvēlīgu pacienta aprūpi un palielinātu medicīnisko kļūdu skaitu.
Introduction: Mental and physical exhaustion is considered to be an occupational hazard, as well as a widespread phenomenon affecting medical staff´s work-abilities, health and the patient safety provided. Consequences due to the exhaustion can be precarious leading to medical errors, impaired medical judgement and decreased effectiveness at work. Aim: The intention with this paper is to analyze the negative impact upon the healthcare workers and their patient care, to research for possible risk factors as well as protective factors. Methods: For assembling of information to conclude for this topic, online databases have been searched such as PubMed, Elsevier, clinical Key, Google scholar, ScienceDirect etc. Selection of the best articles has been chosen and observed, where the main ideas and results has been presented in this paper. Results: When evaluating these numerous articles regarding exhaustion and its adverse effect on patient care, it indicated a positive association between mental and physical exhaustion and increased medical error committed, which had an adverse effect on patient. It was also indicated that medical staff presenting with depression had a higher risk of medical errors. Risk factors contributing to the exhaustion was found to be increased working load and working hours, disturbances of organizational factors, psychosocial work-environment as well as years of experience and young doctors had increased risk of committing medical errors. Protective factors were also evaluated and showed positive effect when intervening in time. It is important to consider and improve organizational factors, regulate working hours and workload among staff as well as to invest in individual factors such as stress management training, mindfulness, self-care and communication skills. These factors have proven to contribute to reduction of the exhaustion among physicians and nurses. Conclusion: In conclusion, out of eleven articles evaluated seven indicated positive association, hence an increased amount of exhaustion is highly associated with adverse patient care and increased medical errors.
