Rezultāti: Vidējais vecums bija 59 gadi ar standarta novirzi 10 gadiem. Dzimumu sa-dalījums bija 14 vīrieši (43,7%) un 18 sievietes (56,2%). Injekciju līmeņi bija šādi: L3-L4 līmenī – 6,25% (n=2), L4-L5 – 65,6% (n=21) un L5-S1 – 28,1% (n=9). ESI/TESI attiecība bija 26 (81,2%) un 6 (18,75%) attiecīgi. Salīdzinot ar sākotnējiem rādītājiem, ievērojams uzlabojums tika novērots NRS un ODI vērtējumos 30., 60., 90. un 180. dienās pēc injekcijas. Laika gaitā uzlabojums vairāk nekā 50% apmērā tika fiksēts šādi: 30. dienā – 78,1% (n=25), 60. dienā – 78,1% (n=25), 90. dienā – 65,6% (n=21) un 180. dienā – 68,7% (n=22). Sākotnējais NRS bija 7,3 ± 1,6 punkti, 30. dienā tas samazinājās līdz 3,0 ± 2,6 punkti-em, 60. dienā līdz 3,4 ± 2,6 punktiem, 90. dienā līdz 3,6 ± 2,5 punktiem un 180. dienā līdz 3,1 ± 2,5 punktiem. Punktu izmaiņas salīdzinājumā ar sākotnējiem datiem bija šādas: līdz 30. dienai – 4,3 ± 2,7 (p<0,001), līdz 60. dienai – 3,8 ± 2,8 (p<0,001), līdz 90. dienai – 3,7 ± 2,6 (p<0,001), līdz 180. dienai – 4,2 ± 2,5 (p<0,001) un no 90. līdz 180. dienai – 0,5 līdz 1,5 (p=0,087). Sākotnējais ODI bija 22 ± 9. 30. dienā tas samazinājās līdz 8 ± 9, 60. dienā līdz 8 ± 8, 90. dienā līdz 10 ± 9 un 180. dienā līdz 9 ± 9. Punktu maiņa no sākotnējā līmeņa līdz 30. dienai bija 13,3 ± 9,2 (p<0,001), līdz 60. dienai – 13,6 ± 8,7 (p<0,001), līdz 90. dienai – 12,0 ± 9,5 (p<0,001), līdz 180. dienai – 13,0 ± 9,7 (p<0,001), un no 90. līdz 180. dienai – 1,0 ± 4,1 (p=0,173). Pārliecības intervāls recidīva iespējamībai 180 dienu laikā bija 50 ± 9% (n=16), ar p-diāporu 33–67% (kļūda p 17%). Visaugstākā recidīva iespējamība tika novērota laika posmā no 60. līdz 90. dienai, sasniedzot 31,25% (n=10). Secinājumi: Fluoroskopijas kontrolē veiktās epidurālās steroīdu injekcijas (ESI) pie-rādījušas savu efektivitāti radikulāro sāpju mazināšanā un pacientu funkcionālās spējas uz-labošanā. Pētījuma rezultāti norāda, ka šāda ārstēšana ievērojami samazina sāpju intensitāti un uzlabo dzīves kvalitāti, kā arī palielina pacientu spēju veikt ikdienas aktivitātes. Sāpju mazināšanās bija ievērojama jau 30. dienā pēc injekcijas, un uzlabojumi saglabājās līdz pat 180. dienai, lai gan turpmākā uzlabojuma dinamika bija lēnāka pēc 90 dienām. Analizējot atkārtotu injekciju nepieciešamību, pētījuma rezultāti liecina, ka optimālais intervāls starp divām injekcijām ir 60 līdz 90 dienas. Šis periods nodrošina labākus rezultātus, samazinot recidīvu risku un uzlabojot ilgtermiņa efektivitāti. Tomēr pētījums norāda uz dažiem recidī-viem, kas liecina par nepieciešamību precīzāk noteikt ideālu injekciju grafiku. Tāpat ir ne-pieciešami turpmāki pētījumi, lai detalizētāk izpētītu ilgtermiņa ietekmi.
The mean age was 59 years with a standard deviation of 10 years. The gender distribution was 14 males (43.7%) and 18 females (56.2%). The injection levels were as follows: L3-L4 – 6.25% (n=2), L4-L5 – 65.6% (n=21), and L5-S1 – 28.1% (n=9). The ESI/TESI ratio was 26 (81.2%) and 6 (18.75%), respectively. Compared to baseline, significant improvement was observed in NRS and ODI scores at 30, 60, 90 and 180 days after injection. Improvements of more than 50% over time were recorded as follows: 78.1% at day 30 (n=25), 78.1% at day 60 (n=25), 65.6% at day 90 (n=21) and 68.7% at day 180 (n=22). The baseline NRS was 7.3 ± 1.6 points, decreasing to 3.0 ± 2.6 points at day 30, 3.4 ± 2.6 points at day 60, 3.6 ± 2.5 points at day 90 and 3.1 ± 2.5 points at day 180. The changes in scores from baseline were as follows: by day 30 – 4.3 ± 2.7 (p<0.001), by day 60 – 3.8 ± 2.8 (p<0.001), by day 90 – 3.7 ± 2.6 (p<0.001), by day 180 – 4.2 ± 2.5 (p<0.001) and from day 90 to 180 – 0.5 to 1.5 (p=0.087). The baseline ODI was 22 ± 9. It decreased to 8 ± 9 at day 30, 8 ± 8 at day 60, 10 ± 9 at day 90, and 9 ± 9 at day 180. The change in points from baseline to day 30 was 13.3 ± 9.2 (p<0.001), to day 60 – 13.6 ± 8.7 (p<0.001), to day 90 – 12.0 ± 9.5 (p<0.001), to day 180 – 13.0 ± 9.7 (p<0.001), and from day 90 to 180 – 1.0 ± 4.1 (p=0.173). The confidence interval for the probability of recurrence within 180 days was 50 ± 9% (n=16), with a p-diameter of 33–67% (error p 17%). The highest probability of recurren-ce was observed in the period from 60 to 90 days, reaching 31.25% (n=10). Conclusions: Fluoroscopy-guided epidural steroid injections (ESI) have proven their effectiveness in reducing radicular pain and improving the functional ability of patients. The results of the study indicate that such treatment significantly reduces pain intensity and im-proves quality of life, as well as increases the ability of patients to perform daily activities. Pain relief was significant already on the 30th day after the injection, and the improvements were maintained up to day 180, although the further dynamics of improvement was slower after 90 days. When analyzing the need for repeated injections, the results of the study indi-cate that the optimal interval between two injections is 60 to 90 days. This period provides better results, reducing the risk of relapse and improving long-term effectiveness. However, the study points to some recurrences, suggesting the need for a more precise definition of the ideal injection schedule. Further studies are also needed to investigate the long-term effects in more detail.
