Bakteriāla endokardīta profilakse pirms stomatoloģiskām procedūrām

Kopsavilkums

Pētījuma mērķis bija izpētīt bakteriāla endokardīta profilakses pielietojumu stomatoloģijas praksē Latvijā un salīdzināt to ar 2015.g. European Society of Cardiology (ESC) vadlīniju rekomendācijām. Šajā šķērsgriezuma pētījumā tika aptaujāti Latvijā praktizējoši stomatologi dažādās specialitātēs (n=34). Tika izveidota elektroniska un papīra formāta anketa, lai izvērtētu stomatologu zināšanas un pieredzi darbā ar augsta bakteriāla endokardīta riska pacientiem. No tiem ESC vadlīnijās ieteiktu antibakteriālo līdzekļu ievades laiku bakteriāla endokardīta pacientiem izvēlējās 64,71% (n=22). Antibakteriālo preparātu, ko iesaka ESC vadlīnijas, savā praksē izvēlējās 55,88% (n=19). Pacientu atlasi, kam būtu nepieciešama antibakteriāla bakteriāla endokardīta profilakse un kas sakrīt ar ESC vadlīnijām, izvēlējās 70,59% (n=14) aptaujāto stomatologu. Procedūras, kurām pēc ESC vadlīniju rekomendācijām būtu nepieciešama antibakteriāla terapija izvēlējās 73,53% (n=25) (ķirurģiska zobu ekstrakcija) un 79,41% (n=27) (zobu implantācija). Bakteriāla endokardīta risku grupas pacientu ārstēšanu uzņemas 100% (n=3) sejas, mutes un žokļu ķirurgi, 83,33% (n=5) periodontologu un tikai 17,39% (n=4) zobārstu. Uztraukumu darbā ar šādiem pacientiem raisa 47,82% (n=11) zobārstiem, 16,67% (n=1) periodontologiem un 100% (n=2) citu specialitāšu stomatologiem. No šādu pacientu ārstēšanas izvairās 17,39% (n=4) zobārstu. Atšķirība starp stomatoloģiskām specialitātēm ir statistiski nozīmīga (p=0,015). Puse no aptaujātajiem stomatologiem savā praksē nenozīmē antibiotikas, to starpā 65,22% (n=15) aptaujāto zobārstu un 100% (n=2) citu specialitāšu pārstāvju. Atšķirība starp specialitātēm ir statistiski nozīmīga (p<0,001).
The aim of the study was to investigate the prevention of infective endocarditis in stomatology in Latvia and to compare it with 2015. for European Society of Cardiology (ESC) guideline recommendations. In this cross-sectional study, dentists practicing in Latvia in various specialties were discussed (n=34). An electronic and paper questionnaire was created to evaluate the knowledge and experience of stomatologists in working with high risk of infective endocarditis. Of these, 64.71% (n=22) chose the time of administration of antibacterial agents recommended by the ESC guidelines for patients with infective endocarditis. The antibacterial drug recommended by ESC guidelines was chosen by 55.88% (n=19) of stomatologists. ESC guidelines recommended patients, who are eligible for antibacterial therapy were chosen by 70,59% (n=14) stomatologists. Procedures that would require antibacterial therapy according to the recommendations of the ESC guidelines were chosen by 73.53% (n=25) (surgical tooth extraction) and 79.41% (n=27) (tooth implantation). 100% (n=3) of facial, oral and maxillofacial surgeons, 83.33% (n=5) of periodontists and only 17.39% (n=4) of dentists take care of patients in the risk group of infective endocarditis. 47.82% of dentists, 16.67% (n=1) of periodontists and 100%(n=2) of stomatologists in other specialities are worried about working with these patients. 17.39% (n=4) dentists avoid such treatment of patients. The difference between stomatology specialties is statistically significant (p=0.015). Half of stomatologists do not administrate antibiotics in their practice, among them are 65.22% (n=15) of surveyed dentists and 100% (n=2) of stomatologists from other specialties. The difference between specialties is statistically significant (p<0.001).
