Aktualitāte: VDV vidū STI izplatība ir augstāka vairāku seksuālās uzvedības faktoru dēļ. STI var arī būt asimptomātiskas, tāpēc nepietiekama STI testēšana noved pie vēlīnas STI atklāšanas un ārstēšanas, kā arī citi faktori, kā zināšanu trūkums un stigmatizācija rada barjeras STI atklāšanai. Šī pētījuma mērķis ir izpētīt STI izplatību un inficēšanās faktorus VDV vidū Baltijas valstīs Hipotēze: STI mūža prevalence statistiski nozīmīgi atšķiras atkarībā no demogrāfiskajiem faktoriem, seksuālās uzvedības faktoriem, STI testēšanos un zināšanām par STI transmisiju. Metodes: Darbs tika izstrādāts, izmantojot datus no EMIS 2017 šķērsgriezuma pētījuma. Izlases lielums bija 834 respondenti. Datu analīzei pielietoja aprakstošās statistikas metodes-biežuma tabulas un šķērstabulas. Veicot Pīrsona Hī-kvadrāta un Fišera Eksakto testu, noteica proporciju atšķirības. Nepārtrauktā mainīgā analīzei veica Manna-Vitnija testu. Statistisko nozīmību noteica, balstoties uz p vērtību (p<0,05). Atšķirības starp proporcijām noteica, balstoties uz īpatsvara 95% ticamības intervālu. Rezultāti: STI mūža prevalence VDV vidū Baltijas valstīs bija 24,0% VDV, kuriem dzīves laikā tika diagnosticēta STI, bija vecāki (38 gadi), nekā VDV, kuriem netika diagnosticēts STI (31 gadi), p<0,001. Visaugstākā STI mūža prevalence vērojama VDV, kuri dzīvoja ļoti lielās–29,0% un vidēja izmēra pilsētās–23,8%, p=0,004. STI mūža prevalence bija augstāka VDV, kuriem pēdējo 12 mēnešu laikā, bija 10 un vairāk nepastāvīgie partneri–42,4%, nekā VDV, kuriem bija neviens, no 1 līdz 3, un no 4 līdz 9 nepastāvīgie partneri, p<0,001. VDV, kuriem bija pastāvīgs partneris, vērojama augstāka STI mūža prevalence–28,4%, nekā VDV, kuriem nebija pastāvīga partnera–20,2%,p=0,009. VDV, kuri nepiekrita, ka viņu seksuālā pieredze ir tik droša, cik paši vēlas, STI mūža prevalence bija augstāka–37,1%, nekā ar VDV, kuri piekrita–22,2%, p<0,001. STI mūža prevalence bija augstāka VDV, kuri veica STI, izņemot HIV, testēšanos dzīves laikā–37,6%, nekā netestēto VDV vidū – 4,9%, p<0,001. Līdzīgi VDV, kuri kādreiz ir veikuši HIV testu, novēroja augstāku STI mūža prevalenci. VDV, kuri zināja, ka inficēšanās ar STI var notikt vieglāk nekā ar HIV, STI mūža prevalence bija augstāka–26,5%, nekā VDV, kuri nezināja šo faktu–18,1%, p=0,009. Līdzīgus rezultātus novēroja attiecībā uz pārējiem apgalvojumiem. Secinājumi: Izvirzītā hipotēze tika daļēji apstiprināta. STI mūža prevalence VDV vidū Baltijas valstīs statistiski nozīmīgi atšķīrās atkarībā no vecuma, apdzīvotības līmeņa, nepatstāvīgo partneru skaita, attiecībām ar pastāvīgu partneri, pašuztveres par seksa drošumu un seksa noraidīšanas kompetenci, STI/HIV testēšanos un zināšanām par STI, bet statistiski nozīmīgi neatšķīrās atkarībā no prezervatīvu lietošanas. Darba saturs: Darbs ir sadalīts nodaļās un apakšnodaļās. Darba apjoms ir 52 lapas. Darbā tika izmantotas 11 tabulas, 3 pielikumi un 65 literatūras avoti.
Topicality: STI prevalence is higher among VDV due to several factors of sexual behavior. STI can also be asymptomatic, which can lead to late STI detection, as well as other factors such as lack of knowledge and stigmatization create barriers to STI detection. The aim of this study is to investigate the prevalence of STI and infection factors among VDV in the Baltic States Hypothesis: Lifetime prevalence of STI differs statistically significantly depending on demographic factors, sexual behavior factors, STI testing and knowledge about STI transmission. Methods: The work was developed using data from the EMIS 2017 cross-sectional study. The sample size was 834 respondents. Descriptive statistical methods - frequency tables and cross-tabulations - were used for data analysis. Differences in proportions were determined by performing Pearson's Chi-square and Fisher's Exact tests. The Mann-Whitney test was performed for the analysis of continuous variables. Statistical significance was determined based on the p value (p<0,05). Differences between proportions were determined based on the 95% confidence interval of the proportion. Results: The lifetime prevalence of STI among MSM in the Baltic States was 24,0%. MSM who were diagnosed with an STI during their lifetime were older (38 years) than MSM who were not diagnosed with an STI (31 years), p<0,001. The highest lifetime prevalence of STI was observed among MSM living in very large–29,0% and medium-sized cities–23,8%, p=0,004. The lifetime prevalence of STI was higher among MSM who had 10 or more non-steady partners in the last 12 months-42,4% than among MSM who had none, 1 to 3, and 4 to 9 non-steady partners, p<0,001. MSM who had a steady partner had a lifetime prevalence of STI-28.4% than MSM who did not have a steady partner-20,2%, p=0,009. VDV who did not agree that their sexual experience was as safe as they wanted it to be had a lifetime prevalence of STI-37.1% than MSM who agreed-22,2%, p<0,001. The lifetime prevalence of STI was higher among MSM who had tested for STI, in their lifetime-37,6% than among untested MSM-4,9%, p<0,001. Similarly, VDV who had tested for HIV had a higher lifetime prevalence of STI. MSM who knew that STI can be contracted more easily than HIV had a lifetime prevalence of STI-26,5% than MSM who did not know this fact-18,1%, p=0,009. Similar results were observed for the other statements. Conclusions: The hypothesis was partially confirmed. The lifetime prevalence of STI among MSM in the Baltic States differed statistically significantly depending on age, population level, number of non-steady partners, relationship with a steady partner, self-perception of safe sex and competence in refusing sex, STI/HIV testing and knowledge about STI, but did not differ statistically significantly depending on condom use.
