Abstract
Mērķi: Darba mērķis ir apskatīt melanomas patofizioloģiskās, histopatoloģiskās un klīniskās atšķirības cilvēkiem ar tumšāku ādas krāsu.
Hipotēze: Melanomas klīniskā izpausme tumšiem ādas fototipiem ir atšķirīga salīdzinājumā ar gaišiem ādas fototipiem.
Materiāli un metode: Šajā literatūras apskatā izvērtēti zinātniskie raksti, kuros aprakstītas melanomas izpausmes cilvēki ar tumšāku ādas krāsu. Šie raksti meklēti elektroniskajās datubāzēs (PubMed, UpToDate, ScienceDirect). Tika izvērtēts rakstu saturs un ticamība, un galu galā šajā darbā tika iekļauti 64 raksti. Tā kā šai tēmai pētniecībā nav pievērsta pietiekama uzmanība, es izmantotajā literatūrā iekļāvu arī sekojošu medicīnas grāmatu: Taylor and Kelly's Dermatology for Skin of Color, 2nd edition.
Rezultāti: Cilvēkiem ar tumšas ādas fototipu, ir atšķirīga melanomas fizioloģija. Melanomas klīniskā izpausme dažādos ādas fototipos ir atšķirīga, savukārt akrālā lentigozā melanoma literatūrā visbiežāk tiek aplūkota tumšo ādas fototipu cilvēkiem. Virspusēji izplatīta melanoma visbiežāk sastopama gaišākiem ādas fototipiem. Arī melanomas riska faktori dažādos ādas fototipos atšķiras.
Secinājums: Trūkst pētījumu, kas aprakstītu dažādu melanomas apakštipu klīnisko izpausmi un melanomas histopatoloģiskās atšķirības tumšo ādas fototipu melanomai. Tas, ka pētījumos nav aprakstīti cilvēki ar tumšāku fototipu, nenozīmē, ka starp gaišajiem fototipiem nav atšķirību, bet drīzāk liecina par šādas tēmas pētījumu trūkumu. Medicīnas speciālistiem ir jābūt labi izglītotiem, lai diagnosticētu melanomu un citas dermatozes dažādos ādas fototipos, jo īpaši tumšākos fototipos, lai nodrošinātu pienācīgu aprūpi saviem pacientiem.
Objectives: The thesis aims to review the pathophysiological, histopathological, and clinical differences of melanoma in skin of color. Hypothesis: Melanoma’s clinical presentation differs in dark skin phototypes compared with light skin phototypes. Materials and method: This literature review evaluates scientific articles that report the manifestations of melanoma in skin of color. These articles are searched from electronic databases (PubMed, UpToDate, ScienceDirect). The content and reliability of the articles were assessed and eventually, 64 articles were included in this thesis. Since the topic has received insufficient attention in the field of research, I also paid attention to the following medical book: Taylor and Kelly’s Dermatology for Skin of Color, 2nd edition. Results: There is a difference in the physiology of melanoma in people with dark phototype. Clinical presentation of melanoma differs between different skin phototypes, whereas acral lentiginous melanoma is mostly viewed in the literature in the dark skin phototypes. Superficial spreading melanoma is the most common in lighter skin phototypes. Also, risk factors for melanoma differ between different skin phototypes. Conclusion: There is a lack of studies describing the clinical presentation of different subtypes of melanoma and histopathological differences in melanoma in dark skin phototypes. People with darker phototype being not described in the studies does not indicate that there is no difference between light phototypes but rather a lack of research around the topic. Medical professionals need to be well educated to diagnose melanoma and other dermatoses in different skin phototypes, especially in darker phototypes, to provide proper care for their patients.
