Ievads un mērķi:
Čūlainais kolīts ir slimība, ko ārstē ar dažādiem medikamentiem, no kuriem viena grupa ir bioloģiskie medikamenti. Tāpat kā jebkuram medikamentam, arī bioloģiskajiem medikamentiem ir blakusparādības. Šī literatūras apskata mērķis ir noskaidrot un izpētīt nevēlamās blakusparādības trīs bioloģiskajos aģentos, proti, infliksimabā, vedolizumabā un ustekinumabā, lietojot tos čūlainā kolīta ārstēšanā.
Metodes:
Literatūras izpētei tika izmantotas Epistemonikos un PubMed datubāzes. Atslēgvārdi bija "blakusparādības", "čūlainais kolīts", "iekaisīga zarnu slimība", "IBD", "bioloģiskie aģenti", "infliksimabs", "vedolizumabs" un "ustekinumabs". Pētījumi, kas atbilda iekļaušanas kritērijiem, tika iekļauti šajā literatūras pārskatā.
Rezultāti:
Infliksimabs ir anti-TNF alfa blokators, kas inhibē TNF-alfa. Vedolizumabs ir anti-integrīnu blokators, kas bloķē integrīnus. Ustekinumabs ir interleikīna 12/23 blokators, kas inhibē šos interleikīnus. Izmantojot šos mehānismus, šie bioloģiskie aģenti nomāc iekaisuma reakciju. Bioloģisko līdzekļu lietošana var izraisīt antivielu veidošanos pret šiem medikamentiem, kas savukārt pēc kāda laika padara zāles neefektīvas. Kopumā septiņi raksti tika atzīti par piemērotiem, lai apspriestu viena vai vairāku iepriekš minēto bioloģisko aģentu blaknes. Sešos no rakstiem tika apskatīts infliksimabs, kā arī citi bioloģiskie aģenti, trīs – vedolizumabs un citi bioloģiskie aģenti, un viens raksts aptvēra tēmu par blakusparādībām, lietojot ustekinumabu čūlainā kolīta ārstēšanā. Turklāt tika iekļauti divi raksti, lai apspriestu antivielu veidošanos pret medikamentiem, kas ir bieži sastopams bioloģisko aģentu blakne.
Secinājumi:
Infliksimabs, vedolizumabs un ustekinumabs ir bioloģiskie aģenti ar trīs dažadiem darbības mehānismiem, kuriem vajadzētu nomākt iekaisuma reakciju, tādējādi efektīvi ārstējot čūlaino kolītu, kā arī citas slimības. Jāapzinās, ka, lietojot bioloģiskos aģentus, pastāv iespēja veidoties antivielām pret šiem medikamentiem, un, ja šādos gadījumos zāles vairs nebūs tik efektīvas. Lai gan ir ļoti labi, ka šie medikamenti pastāv, tos lietojot, jābūt īpaši uzmanīgiem, jo tiem ir raksturīgas blaknes un neatkarīgi no smaguma pakāpes šīs parādības ir jāuztver nopietni.
Background and aims: Ulcerative colitis is a disease treated with various medical drugs, one of them being biological agents. As with every drug, with an effect comes a side effect. The aim of this literature review is to find out and investigate adverse events in three biological agents, namely infliximab, vedolizumab and ustekinumab, when used in treatment of ulcerative colitis. Methods: Epistemonikos and PubMed was databases used for the literature research. Keywords were “side effects”, “adverse effects”, “adverse events”, “adverse reactions”, “ulcerative colitis”, “inflammatory bowel disease”, “IBD”, “biological agents”, “infliximab”, “vedolizumab” and “ustekinumab”. The studies that met the inclusion criteria were included in this literature review. Results: Infliximab is an anti – TNF alpha blocker, which inhibits TNF – alpha. Vedolizumab is an anti – integrin blocker, which blocks integrins. Ustekinumab is an interleukin 12/23 blocker, which inhibits these interleukins. Through these mechanisms, these biological agents suppress the inflammation response. The use of biological agents, may induce formation of antidrug – antibodies, which in turn makes the drug inefficient after some time. In total, seven articles was found eligible to discuss the adverse events of one or more of the previously mentioned biological agents. Six of the articles covered infliximab, amongst other biological agents, three covered vedolizumab, amongst other biological agents, and one article covered the topic of adverse events in ulcerative colitis when using ustekinumab. In addition, two articles were included to discuss antidrug – antibodies, which is a common adverse event of biological agents. Conclusions: Infliximab, vedolizumab and ustekinumab are three different acting biological agents that should normally result in the suppression of the inflammatory response, thereby being efficient in treatment ulcerative colitis, amongst other diseases. One should be aware that there is a possibility of developing antidrug – antibodies when using biological agents, and if this event occurs, the drug will not be as efficient anymore. Although it is very good that these drugs exist, one should be extra careful when using them, because adverse events might occur as a complication of the drug, and regardless of the severity, these events should be taken seriously.
