Vienā no Latvijas V līmeņa ārstniecības iestādēm operāciju zālēs novērots, ka pacienti pirmsoperācijas periodā nereti ir nepietiekami sagatavoti plānveida, tostarp atkārtotām operācijām, kā rezultātā operācijas tiek atceltas. Šādai situācijai ir būtiskas sekas – tā ietekmē veselības aprūpes izmaksas, pieejamību un piekļuvi šim pakalpojumam, kā arī ietekmē veselības aprūpes kvalitāti.
Kā biežākie operācijas atcelšanas iemesli literatūrā tiek minēti pacienta veselības stāvoklis, laika trūkums un organizatoriskie faktori (Naderi-Boldaji et al, 2023). Sawadago et al. pētījums Burkina-Faso uzsver, ka lielākā daļa operāciju atcelšanas gadījumu ir novēršami un saistīti ar slimnīcas darba organizāciju (Sawadago et al, 2022).
Līdz šim šajā konkrētajā V līmeņa ārstniecības iestādē nav veikts padziļināts izvērtējums par faktoriem, kas ietekmē plānveida operāciju atcelšanu. Plānveida operāciju atcelšana ir problēma globālā mērogā, to bieži izraisa laika resursu trūkums, administratīvie jautājumi, kā arī operāciju telpu un aprīkojuma trūkums. Plānveida operāciju atcelšana ir neefektīva ķirurģiskā laika izmantošana un būtiska veselības aprūpes resursu izšķērdēšana. Jaunattīstītajās valstīs ar ierobežotiem resursiem, sliktu sociālekonomisko apstākļu dēļ, lielākā daļa slimnīcu saskaras ar biežu plānveida operāciju atcelšanu (Ayalew, 2024).
Pētījuma mērķis ir novērtēt plānveida operāciju atcelšanas apmēru un ar to saistītos faktorus Latvijas V līmeņa ārstniecības iestādes stacionāros, identificējot galvenos cēloņus un ietekmes faktorus.
Pētījuma dizains paredzēts kā kvantitatīvs, neeksperimentāls, prospektīvs un aprakstošs. Prospektīvā novērošanas ietvaros tiks apzināti biežākie plānveida operāciju atcelšanas iemesli katrā no pētījumā iekļautajām V līmeņa ārstniecības iestādes stacionāriem. Datu apstrādei izmantotas aprakstošo un salīdzinošo statistisks metodes, analizējot atkarīgo un neatkarīgo mainīgo biežumu, ņemot vērā stacionāru specifiku, ķirurģiskā profila īpatnības un operāciju plānojuma aspektus.
Datu ieguvei izmantoti divi instrumenti, no kuriem viens paredzēts pacientiem, otrs plānveida atcelto operāciju apkopošanai.
Pētījumā izvirzīta hipotēze - galvenie faktori, kas ietekmē plānveida operāciju atlikšanu ir pacienta pirmsoperācijas sagatavošanas kļūdas, nevis pacienta personīgie faktori vai organizatoriskie jautājumi.
In the operating rooms of one of Latvia's level V healthcare institutions, it has been observed that patients are often insufficiently prepared during the preoperative period for elective surgeries, including repeated procedures, resulting in cancellations. This situation has significant consequences—it affects healthcare costs, the availability and accessibility of services, as well as the overall quality of care. The most commonly cited reasons for surgery cancellations in the literature are the patient's health condition, time constraints, and organizational factors (Naderi-Boldaji et al., 2023). A study by Sawadago et al. in Burkina Faso highlights that the majority of surgical cancellations are preventable and related to hospital workflow and organization (Sawadago et al., 2022). To date, no in-depth evaluation has been conducted in this particular level V institution to assess the factors influencing the cancellation of elective surgeries. The cancellation of elective surgeries is a global issue, often attributed to limited time resources, administrative barriers, and shortages of operating rooms and equipment. Such cancellations result in inefficient use of surgical time and significant waste of healthcare resources. In developing countries with constrained resources and poor socioeconomic conditions, hospitals frequently face high rates of elective surgery cancellations (Ayalew, 2024). The aim of this study is to evaluate the extent and contributing factors of elective surgery cancellations in inpatient departments of a Latvian level V healthcare institution, with a focus on identifying key causes and influencing factors. The study design is quantitative, non-experimental, prospective, and descriptive. Through prospective observation, the most common reasons for elective surgery cancellations will be identified in each of the inpatient departments included in the study. Descriptive and comparative statistical methods will be used to process the data, analyzing the frequency of dependent and independent variables, while taking into account the specifics of the departments, surgical profile characteristics, and scheduling aspects. Two instruments will be used for data collection—one designed for patients and the other for documenting canceled elective surgeries. The study hypothesizes that the primary factors influencing the postponement of elective surgeries are related to errors in the patient's preoperative preparation, rather than personal patient-related or organizational factors.
