Zobu neizšķilšanās ir patoloģisks stāvoklis, kurā zobs neiekļaujas zobu rindā paredzētajā laikā vietējo vai sistēmisko faktoru dēļ. Lai gan trešo molāru neizšķilšanās ir bieži sastopama parādība, pirmo un otro pastāvīgo molāru neizšķilšanās ir ievērojami retāka un bieži vien netiek pietiekami dokumentēta. Neskatoties uz to retumu, šie gadījumi rada būtiskas diagnostiskas un terapeitiskas grūtības.
Šajā literatūras apskatā tiek pētīta pirmo un otro molāru neizšķilšanās etioloģija, diagnostiskie apsvērumi un ārstēšanas stratēģijas. Tika veikta plaša recenzētu zinātnisko rakstu meklēšana datubāzēs, piemēram, PubMed, Google Scholar, ScienceDirect un ProQuest. Apskatā analizēti dati par izplatību, klasifikāciju, radioloģiskajiem atradumiem un ārstēšanas iespējām, tostarp ortodontisko iztaisnošanu, ķirurģisku iejaukšanos un ekstrakciju.
Rezultāti liecina, ka agrīna diagnostika, ko veicina radioloģiskā izmeklēšana un klīniska uzraudzība, būtiski uzlabo ārstēšanas rezultātus. Diagnostikas rīki, piemēram, CBCT, uzlabo precizitāti un atbalsta individuālu ārstēšanas plānošanu. Ārstēšanas panākumi ir atkarīgi no pacienta individuālajiem faktoriem, tostarp vecuma, zoba pozīcijas un ankiloze vai telpas ierobežojumu klātbūtnes.
Šis darbs uzsver nepieciešamību pēc lielākas informētības zobārstniecības speciālistu vidū par molāru izšķilšanās traucējumiem un atbalsta preventīvu izšķilšanās novērtējumu iekļaušanu ikdienas klīniskajā praksē.
Tooth impaction is a pathological condition in which a tooth fails to erupt into the dental arch within the expected timeframe due to local or systemic factors. While third molar impactions are commonly encountered, impactions of the first and second permanent molars are significantly less frequent and underreported. Despite their rarity, these cases pose considerable diagnostic and therapeutic challenges. This literature review explores the etiology, diagnostic considerations, and treatment strategies for the impaction of first and second molars. A comprehensive search of peer-reviewed articles was conducted using databases such as PubMed, Google Scholar, ScienceDirect, and ProQuest. The review analyzes data regarding the prevalence, classification, radiographic findings, and management options, including orthodontic uprighting, surgical interventions, and extraction. Findings indicate that early detection, facilitated by radiographic evaluation and clinical monitoring, significantly improves treatment outcomes. Diagnostic tools such as CBCT enhance accuracy and support individualized care planning. Treatment success depends on patient-specific factors, including age, tooth position, and the presence of ankylosis or space constraints. This thesis emphasizes the need for increased awareness among dental professionals regarding molar eruption disturbances and supports the integration of preventive eruption assessments into routine clinical practice
