Ierobežojumi aktivitātes un dalības komponentēs Starptautiskā funkcionēšanas klasifikācijā cilvēkiem ar muguras smadzeņu bojājumu, izmantojot WHODAS 2.0 novērtēšanas instrumentu

Abstract
Darba mērķis: noskaidrot ierobežojumus, ar kuriem saskaras cilvēki ar muguras smadzeņu bojājumu, Starptautiskās Funkcionēšanas klasifikācijas komponentēs “Aktivitāte un dalība.”
Darba jautājums: vai cilvēki ar muguras smadzeņu bojājumu saskaras ar viegliem, mēreniem, smagiem vai absolūtiem aktivitāšu un dalības ierobežojumiem?
Pētījuma norise: neeksperimentālā kvantitatīvā pētījumā piedalījās 14 respondenti, atlase netika veidota epidemioloģiskās situācijas dēļ. Instrumentam tika izvēlēta WHODAS 2.0 anketa, 36 punktu versija, respondenta forma, kas tika izplatīta caur Internetu sadarbībā ar vairākām ar invaliditāti saistītām biedrībām. Rezultāti tika analizēti ar aprakstošās statistikas metodēm, izmantojot SPSS, Microsoft Excel, Microsoft Power BI programmas.
Rezultāti: tika noskaidrots ierobežojumu lielums sešos uz Starptautiskās Funkcionēšanas klasifikācijas balstītos domēnos. Pētījuma dalībnieku aktivitāšu un dalības ierobežojumi, analizējot pēc WHODAS 2.0 sarežģītās vērtēšanas, bija vienādi ar 70,09 domēnā “Mobilitāte”, 52,08 “Līdzdalība sabiedrībā” domenā, 51,19 “Ikdienas dzīves aktivitātes” domēnā, 40 “Pašaprūpe” domēnā, 33,33 “Saprašanās ar cilvēkiem” domēnā, 12,86 “Izpratne un komunikācija” domēnā. Tika iegūti arī vienkāršās vērtēšanas rezultāti katram domēnam un katram jautājumam domēnā. Pētījumā tika noteikta statistiski nozīmīga lineārā korelācija starp muguras smadzeņu bojājuma smaguma pakāpi pēc ASIA bojājuma skalas un domēna “Mobilitāte” un domēna “Pašaprūpe” aktivitāšu ierobežojumiem.
Secinājumi: cilvēki ar muguras smadzeņu bojājumu saskaras ar viegliem ierobežojumiem domēnā “Izpratne un komunikācija”; mēreniem ierobežojumiem domēnā “Pašaprūpe” un “Saprašanās ar cilvēkiem”; smagiem ierobežojumiem domēnā ”Mobilitāte”, “Dzīves aktivitātes”, “Līdzdalība sabiedrībā”. Ir atrasta sakarība starp muguras smadzeņu bojājuma pakāpi pēc ASIA skalas un domēniem “Mobilitāte” un “Pašaprūpe” – jo smagāka ir bojājuma pakāpe pēc ASIA skalas, jo lielāki ir ierobežojumi šajos domēnos.
Objective: to identify the limitations faced by people with spinal cord injury in the Activity and Participation components of the International Classification of Functioning. Research question: the study answers the question of whether people with spinal cord injuries face no, mild, moderate, severe, or complete limitations in activity and participation components of the International Classification of Functioning. Design: 14 respondents participated in the non-experimental quantitative study; the selection was not made due to the epidemiological situation. The WHODAS 2.0 36-item, self-administered questionnaire, which was distributed via the Internet in cooperation with several disability-related associations, was chosen as the study instrument. The results were analyzed by descriptive statistical methods using SPSS, Microsoft Excel, Microsoft Power BI programs. Results: activity and participation limitation severity in six domains based on the International Classification of Functioning was determined. The average activity and participation limitations were analyzed according to the WHODAS 2.0 complex scoring and were equal to 70,09 in the “Mobility” domain, 52,08 in the “Participation” domain, 51,19 in the “Life activities” domain, 40 in the “Self-care” domain, 33,33 in the “Getting along with people” domain, 12,86 in the “Understanding and communicating” domain. Simple scoring results were also obtained for each domain and for each question in the domain. The study found a statistically significant linear correlation between the severity of spinal cord injury according to the ASIA impairment scale and the activity limitations in the “Mobility” and “Self-care” domains. Discussion: people with spinal cord injury face mild activity limitations in “Understanding and communicating” domain; moderate limitations in domains: “Self-care” and “Getting along with people”; severe limitations in domains: “Getting around”, “Participation in society”, “Life activities”. A relationship has been found between the severity of spinal cord injury according to the ASIA impairment scale and the domains “Mobility” and “Self-care” – the more severe the degree of damage according to the ASIA impairment scale, the greater the limitations in these domains.
