Comparison of Endovascular Treatment and Burr Hole Drainage in Chronic Subdural Hematoma

Abstract
Hroniskas subdurālās hematomas mūsdienās ir diezgan izplatīts neiroķirurģisks stāvoklis. Tradicionālā ārstēšanas metode ir hematomas ķirurģiska evakuācija (caurumu drenāža). Tomēr šai metodei ir salīdzinoši augsts recidīvu un komplikāciju biežums. MMA embolizācija ir jauna ārstēšanas metode hroniskas subdurālās hematomas ārstēšanā. Šajā rakstā ir salīdzināta caurumu drenāžas un MMA embolizācijas efektivitāte. Piko metode tika izmantota, lai veiktu sistemātisku meklēšanu, un pēc 14 ar šīm procedūrām saistītu rakstu analīzes tika konstatēts, ka MMA embolizācija ir efektīva un drošāka alternatīva caurumu drenāžai ar ievērojami zemāku recidīvu biežumu un saslimstību. Ir nepieciešami turpmāki pētījumi, lai noteiktu tā ilgtermiņa rezultātus un rentabilitāti salīdzinājumā ar caurumu drenāžu, kā arī noteiktu mirstības atšķirību starp abām metodēm.
Chronic subdural hematomas are a fairly common neurosurgical condition in current times. The traditional treatment modality is surgical evacuation (burr-hole drainage) of the hematoma. However this method has a relatively high recurrence rate and complications. MMA embolization is a novel treatment method in the treatment of chronic subdural hematoma. This paper compares the efficiency of burr-hole drainage and MMA embolization. The pico method was used to carry out a systematic search and after analyzing 14 articles related to these procedures it was found that MMA embolization is an efficient and safer alternative to burr-hole drainage, with a significantly lower recurrence rate and morbidity. Further research is required to determine its long-term outcomes and cost-effectiveness compared to burr-hole drainage and as well as to determine the difference in mortality between the two methods.
