Pamatojums: Ar gūžas adduktoriem saistītas cirkšņa sāpes ir izplatīta problēma sportistu
vidū. Tās veido lielāko daļu no visām cirkšņa sāpēm, un bieži ir ilgstošas. Galvenie riska faktori
ir gūžas adduktoru vājums un muskuļu disbalanss. Lai sniegtu pamatotus secinājumus par
terapeitisko vingrinājumu efektivitāti ar gūžas adduktoriem saistītu cirkšņa sāpju mazināšanā un
gūžas locītavas funkciju uzlabošanā, nepieciešams sistemātisks literatūras pārskats, kas apkopotu
un kritiski izvērtētu kvalitatīvus jaunākos pētījumus.
Pētījuma mērķis: Noskaidrot terapeitisko vingrinājumu efektivitāti sāpju mazināšanā un
gūžas locītavas funkciju uzlabošanā sportistiem ar gūžas adduktoriem saistītām ilgstošām cirkšņa
sāpēm.
Pētījuma metodes: Pētījuma dizains ir sistemātisks literatūras pārskats. Pētījumu
identificēšana veikta sešās datubāzēs: PubMed, Science Direct, EBSCO, Sage Journals,
Cochrane un Scopus. Izmantojot meklēšanas stratēģiju, tika identificēti 395 pētījumi.
Rezultāti: Pētījumā iekļauti un analizēti pieci nejaušināti kontrolēti pētījumi. Pētījumos
izmantotas dažādas vingrinājumu programmas, kas iekļauj gūžas adduktoru spēka un stiepšanās
vingrinājumus, vēderpreses un muguras muskuļu spēka vingrinājumus, līdzsvara un koordinācijas
vingrinājumus. Pētījumu dalībnieki uzrādīja statistiski nozīmīgu sāpju samazinājumu un gūžas
locītavas funkciju uzlabojumu, pielietojot dažādas vingrinājumu programmas. Visos pētījumos
tika sasniegtas minimālās klīniski nozīmīgās izmaiņas Kopenhāgenas gūžas un cirkšņa iznākuma
vērtējuma skalā, kas liecina par funkcionālo un ar veselību saistītās dzīves kvalitātes uzlabošanos.
Secinājumi: Strukturēti un progresējoši terapeitiskie vingrinājumi ir efektīvi cirkšņa sāpju
mazināšanā, gūžas locītavas funkciju un ar veselību saistītas dzīves kvalitātes uzlabošanā
sportistiem ar ilgstošām ar gūžas adduktoriem saistītām cirkšņa sāpēm. Papildus ieguvumus ar
veselību saistītās dzīves kvalitātes uzlabošanā sniedz gūžas adduktoru spēka vingrinājumu
iekļaušana vingrinājumu programmā, sevišķi ekscentriskā muskuļu kontrakciju režīmā.
Title: The Effectiveness of Therapeutic Exercises in Reducing Long-Lasting Adductor-related Groin Pain and Improving Hip Joint Functions in Athletes: A Systematic Review Background: Hip adductor-related groin pain is a common issue among athletes. It accounts for the majority of all groin pain cases in athletes and is frequently characterized by a chronic course. The main risk factors include hip adductor weakness and muscle imbalance. To provide evidence-based conclusions on the effectiveness of therapeutic exercises in reducing hip adductor-related groin pain and improving hip joint functions, a systematic literature review is necessary to compile and critically evaluate high-quality recent studies. Objective: To evaluate the effectiveness of therapeutic exercise interventions in alleviating pain and enhancing hip joint function in athletes with chronic hip adductor-related groin pain. Methods: The study design is a systematic literature review. Researches were identified across six databases: PubMed, Science Direct, EBSCO, Sage Journals, Cochrane, and Scopus. Using the search strategy, 395 studies were identified. Results: Five randomized controlled trials were included and analyzed. These studies used various exercise programs that incorporated hip adductors strengthening and stretching exercises, core and back muscle strengthening, as well as balance and coordination exercises. Participants showed statistically significant reductions in pain and improvements in hip joint function following different exercise interventions. All studies achieved the minimum clinically important difference on the Copenhagen Hip and Groin Outcome Score, indicating improvements in functions and health-related quality of life. Conclusions: Structured and progressive therapeutic exercise programs are effective in reducing groin pain, improving hip joint functions and enhancing health-related quality of life in athletes with long-lasting hip adductor-related groin pain. Additional benefits in health-related quality of life are achieved when hip adductor strengthening exercises, particularly eccentric muscle contractions, are included in the exercise program.
