Uzturs ar zemu ogļhidrātu saturu (GAPS) bērniem ar uzmanības deficītu un hiperaktivitāti

Abstract
Pētījuma mērķis bija izpētīt, vai ar GAPS diētas vadlīnijās balstītu uzturu un prebiotiķi Happy Tummy ar Hufulac iespējams mazināt uzmanības deficīta un hiperaktivitātes simptomus bērniem.
Pēc Amerikas Savienoto Valstu (ASV) Slimību kontroles un profilakses centra datiem, ASV 9,4 % bērnu tiek diagnosticēti ar uzmanības deficīta un hiperaktivitātes sindromu (UDHS) (CDC, 2020). UDHS simptomi var izpausties kā pārmērīgi liels kustīgums (hiperaktivitāte), grūtības noturēt uzmanību vai kontrolēt impulsīvu uzvedību. Bērniem ar UDHS bieži ir arī gremošanas trakta darbības traucējumi, kā arī izmainīts zarnu mikrobioms (Ming et al., 2018). Aizvien pieaug pētījumu skaits par uztura terapijas potenciālu UDHS simptomu mazināšanā (Pelsser et al., 2017; Rytter et al., 2015; Stevenson et al., 2014).
Šis bija trīs mēnešus ilgs kvantitatīvs, prospektīvs, gadījuma-kontroles uztura intervences pētījums, kas ietvēra zema ogļhidrātu satura uzturu (Gut and Psychology Syndrome diet, GAPS), kā arī prebiotiķi Happy Tummy ar Hufulac. Pētījumā piedalījās 50 bērni ar UDHS diagnozi un/vai ar uzmanības deficīta sindroma (UDS) diagnozi vai ar šīm diagnozēm atbilstošajiem simptomiem, bet vēl bez diagnozes. Bērni bija vecumā no pieciem līdz 13 gadiem, no tiem 49 dzīvo Latvijā, viens bērns dzīvo Zviedrijā. 19 Bērni bija GAPS grupā, 19 bērni - GAPS+Prebiotiķis grupā, un 12 bērni bija Kontroles grupā. Bērnu UDHS un gremošanas simptomus četras reizes pētījuma laikā novērtēja bērnu vecāki, kā arī bērni paši, izmantojot septiņas adaptētas vai validētas anketas. Bērnu fēču analīzes tika izvērtētas pētījuma sākumā un beigās.
Trīs mēnešu GAPS un GAPS + Prebiotiķa Happy Tummy ar Hufulac uztura intervence būtiski samazināja UDHS simptomus bērniem vecāku vērtējumā (p < 0,001), kā arī samazināja bērnu UDHS simptomus (p = 0,002) un hroniska stresa izpausmes (p = 0,036) bērnu skatījumā. Statistiski ticamas izmaiņas gastrointestinālo simptomu samazinājumā trīs mēnešu GAPS un GAPS + Prebiotiķa Happy Tummy uztura intervences laikā grupu ietvaros un starp grupām netika novērotas, taču relatīvais vidējais simptomu samazinājums intervences grupās sasniedza 50 %. Fēču analīzes apstiprināja, ka bērniem ar UDHS gremošanas trakta funkcijas nav optimālas – bērniem bija zarnu disbioze un apgrūtināta makro-uzturvielu sagremošana. Prebiotiķa Happy Tummy ar Hufulac pozitīva ietekme uz bērnu UDHS simptomu samazinājumu papildus GAPS uzturam netika novērota. GAPS diēta var būt droša un efektīva pieeja UDHS simptomu mazināšanai bērniem.
The objective of the study was to investigate the potential of low carbohydrate diet (Gut and Psychology Syndrome diet, GAPS) and prebiotic Happy Tummy with Hufulac in reducing symptoms of attention deficit and hyperactivity in children. According to Centers for Disease Control and Prevention, 9.4 % of children in the USA have been diagnosed with attention deficit and hyperactivity disorder (ADHD) (CDC, 2020). Symptoms of ADHD may include trouble paying attention, controlling impulsive behaviours, or being overly active. Children with ADHD often have underlying digestive problems and altered gut microbiome (Ming et al., 2018). The growing research body is looking at nutritional and dietary interventions as treatment for ADHD symptoms (Pelsser et al., 2017; Rytter et al., 2015; Stevenson et al., 2014). The present study involved a quantitative, case-control 3-month treatment of a nutritional and dietary intervention involving a low carbohydrate diet (GAPS) and the use of a prebiotic Happy Tummy with Hufulac. The participants were 50 children (five to 13 years of age) from Latvia (49) and Sweden (1) with diagnoses of attention deficit disorder (ADD) or/and ADHD or with undiagnosed ADD/ADHD symptoms. Out of 50 children, 19 children were in the GAPS group, 19 in the GAPS+Prebiotic group, and 12 children were in the Control group. Symptoms of ADHD and digestive symptoms of children were evaluated by parents and children themselves using seven adapted or validated questionnaires four times during the study. Faecal analysis of children was evaluated at the beginning and end of the study. Results of the study demonstrate that three-month intervention of GAPS diet and prebiotic Happy Tummy with Hufulac significantly reduces the symptoms of ADHD in children according to parent evaluation (p < 0.001) and reduces the symptoms of ADHD (p = 0.002) and chronic stress (p = 0.036) according to children’s own evaluation. Statistically significant reduction of gastrointestinal symptoms in children during the study was not observed for GAPS and GAPS+Prebiotic groups or among the three groups, but the relative reduction of symptoms for both intervention groups reached 50 %. The functions of gastrointestinal tract of children with ADHD symptoms at base were not optimal - children had gut dysbiosis and impaired digestion of macronutrients. Results did not show increased effect size for the treatment with GAPS diet and prebiotic Happy Tummy with Hufulac. GAPS diet can be a safe and effective approach to help reduce symptoms of ADHD in children.
