Darba nosaukums. Optimālākais ekscentrisku spēka vingrojumu modelis Ahileja cīpslas funkciju atjaunošanā personām ar hronisku tendinopātiju: sistemātisks literatūras apskats.
Pamatojums. Katru gadu hroniska Ahileja cīpsla vidusdaļas tendinopātija tiek diagnosticēta 6,2 - 9,5% aktīva dzīvesveida piekritējiem. Ahileja cīpslas funkciju atjaunošanā visbiežāk tiek pielietota ekscentrisku spēka vingrinājumu terapija. Pētījumos pastāv dažādas variācijas par vingrojumu izpildi, bet līdz galam nav noskaidrots optimālākais terapijas modelis. Izraugoties optimālāko terapijas modeli, būtu iespēja sekmīgāk atgūt cīpslas funkciju un izvairīties no neatgriezeniska cīpslas bojājuma, kas noved pie operatīvas iejaukšanās.
Mērķis. Apkopot un analizēt pētījumus par optimālāko ekscentrisku spēka vingrinājumu modeli Ahileja cīpslas funkciju atjaunošanā personām ar hronisku tendinopātiju.
Metodes. Publikāciju meklēšana veikta PubMed, PEDro, EBSCOhost, Science Direct un Embase tiešsaistes datubāzēs pēc iepriekš izraudzītiem atslēgas vārdiem eccentric; Achilles tendinopathy; tendon; treatment; exercise therapy un training. Darbā iekļauti randomizēti kontrolēti pētījumi ar atšķirīgiem ekscentrisku spēka vingrinājumu terapijas modeļiem. Atlasītie pētījumi ir savstarpēji salīdzināti un veikta datu analīze par pielietoto ekscentrisku spēka terapijas modeli un pētījuma rezultātiem.
Rezultāti. Pētījumu daļā analizētas piecas publikācijas ar atšķirīgiem ekscentrisku spēka vingrinājumu terapijas modeļiem un kopumā tika iesaistīti 198 dalībnieki. Trīs pētījumos tika novērots vienlīdz klīniski nozīmīgs uzlabojums pēc ekscentrisku spēka vingrinājumu terapijas pielietošanas. Divos pētījumos pielietotā ekscentrisku vingrojumu terapija rezultējas ar maznozīmīgiem uzlabojumiem Ahileja cīpslas funkciju atjaunošanā.
Secinājumi. Paaugstinātas pretestības terapijas modelis, lineāri palielinot slodzi un samazinot atkārtojumu skaitu, rezultējas ar klīniski nozīmīgiem uzlabojumiem. Modificēts Alfredsona terapijas modelis ar samazinātu nodarbību biežumu un modificēts Alfredsona terapijas modelis, kur rekomendē veikt vingrojumus ar maksimāli iespējamu atkārtojumu skaitu vienā piegājienā, rezultējas ar vienlīdz klīniski nozīmīgiem uzlabojumiem Ahileja cīpslas funkciju atjaunošanā.
Title. The optimal model of eccentric strength exercises in improving Achilles tendon function in persons with chronic tendinopathy: a systematic review of the literature. Background. Every year chronic mid-portion Achilles tendinopathy is diagnosed in 6,2 - 9,5% of active lifestyle supporters. Eccentric strength exercises are the most commonly used therapy to improve Achilles tendon function. There are various variations in the performance of exercises in the studies, but the most optimal treatment model has not been determined yet. By choosing the optimal treatment model, it would be possible to more successfully recover tendon function and avoid irreversible tendon damage that leads to operative intervention. Objective. Collect and analyze information on the optimal eccentric strength exercise model of Achilles tendon function improvement in persons with chronic tendinopathy. Methods. Publications search were performed in PubMed, PEDro, EBSCOhost, Science Direct and Embase online databases by pre-selected keywords eccentric; Achilles tendinopathy; tendon; treatment; exercise therapy and training. The study includes randomized controlled trials with different models of eccentric strength exercise therapy. The selected studies have been compared and data analysis has been performed on the applied model of eccentric strength exercise therapy. Results. Five publications with different models of eccentric strength exercise therapy were analyzed and a total of 198 participants were involved in this review. In three studies, clinically significant improvement was observed after the use of eccentric strength exercise therapy. In two studies, minor improvement of Achilles tendon function was observed after the use of eccentric strength exercise therapy. Conclusions. Heavy load resistance exercise therapy results in clinically significant improvements by linearly increasing the load and decreasing the number of repetitions. A modified Alfredson therapy model with a reduced frequency of exercises and a modified Alfredson therapy model which recommends performing exercises with the maximum possible number of repetitions in one time, result in equally clinically significant improvements in Achilles tendon function.
