Agrīnu anestēzijas komplikāciju un pacientu riska faktoru retrospektīva analīze bērniem pēc adenotomijas vai tonsilektomijas operācijas VSIA "Bērnu klīniskajā universitātes slimnīcā"

Adenotomija un tonsilektomija ir bieži veiktas ķirurģiskas procedūras pediatrijas populācijā kā ārstēšanas taktika rekurentu akūtu tonsilītu, obstruktīvas miega apnojas (OMA) un hronisku adenoidītu gadījumā. Šīm operācijām ir nepieciešama vispārējā anestēzija, kas var būt saistīta ar agrīnām pēcoperācijas anestēzijas komplikācijām. Pētījuma mērķis bija noteikt agrīnu anestēzijas komplikāciju un pacientu riska faktoru korelācijas bērniem vecumā no 0 līdz 7 gadiem pēc adenotomijas vai tonsilektomijas operācijas VSIA “Bērnu klīniskā universitātes slimnīca”. Pētījumā tika analizētas 8378 medicīniskās vēstures laika posmā no 2014. gada 1. janvāra līdz 2024. gada 31.decembrim pacientiem, kuriem tika veikta adenotomija vai tonsilektomija akūtā vai plānveida kārtā VSIA “Bērnu klīniskā universitātes slimnīca”. Analīzē tika iekļauti tādi dati kā pacientu vecums, dzimums un agrīnas pēcoperācijas komplikācijas, kā arī iespējami riska faktori anamnēzē. Pētījumā sākotnēji tika atlasīti 19 pacienti, kuriem bija konstatētas agrīnas pēcoperācijas komplikācijas, taču 7 tika izslēgti, kā rezultātā gala izlasē bija 12 pacienti, iegūstot pēcoperācijas agrīnu anestēzijas komplikāciju incidenci 0,14%. Pētījuma ietvaros tika konstatētas tādas pēcoperācijas agrīnas anestēzijas komplikācijas kā laringospazma ar incidenci 0,06%, bronhospazma ar incidenci 0,012%, pēcoperācijas slikta dūša un vemšana (PONV) ar incidenci 0,012% un mutes dobuma orgānu bojājums ar incidenci 0,012%. Ņemot vērā nelielo atlases skaitu (n=12), nebija iespējams veikt aprēķinu, izmantojot Spīrmena ranga korelācijas koeficientu (ρ). Tomēr veicot Fišera eksakto testu divām biežākajām komplikācijām – laringospazma un bronhospazma, netika konstatēta statistiski nozīmīga atšķirība starp vecuma grupām 0-3 gadu vecums un 4-7 gadu vecums. Šie rezultāti liecina, ka agrīnas pēcoperācijas anestēzijas komplikācijas nav tieši atkarīgas no pacienta vecuma.
Adenotomy and tonsillectomy are commonly performed surgical procedures in the pediatric population as a treatment strategy for recurrent acute tonsillitis, obstructive sleep apnea (OSA), and chronic adenoiditis. These surgeries require general anesthesia, which are associated with early postoperative anesthetic complications. The aim of this study was to determine the correlation between early onset anesthesia complications and patient risk factors in children aged from 0 to 7 years after adenotomy or tonsillectomy at the Children’s Clinical University Hospital. The study analyzed 8378 medical records from patients that underwent adenotomy or tonsillectomy, either as an elective or emergency procedure, at the Children’s Clinical University Hospital between January 1, 2014, and December 31, 2024. Patient data, including age, sex, and early onset postoperative anesthesia complications, were analyzed and compared, as well as potential risk factors were identified in the medical record. Initially, 19 patients with identified anesthesia complications were included in the study. However, 7 patients were excluded based on the exclusion criteria, leaving a final cohort of 12 patients, and yielding an incidence of early postoperative anesthesia complications of 0.14%. Early onset anesthesia complications were identified in the study – laryngospasm with an incidence of 0.06%, bronchospasm with an incidence of 0.048%, postoperative nausea and vomiting (PONV) with an incidence of 0.012%, and oral cavity organ injury with an incidence of 0.012%. Given the small sample size (n=12), it was not possible to calculate the correlation coefficient using the Spearman’s rank correlation coefficient (ρ). However, when performing the Fisher’s exact test for the two most common complications – laryngospasm and bronchospasm – no statistically significant difference was found between age groups 0-3 years and 4-7 years. These results suggest that early onset postoperative anesthesia complications are not directly dependent on patient age.
