Pedofīli traucējumi (PeD) ir sarežģīts psihisks stāvoklis, ko raksturo ilgstoša seksuāla interese par bērniem pirmspubertātes vecumā. Šajā literatūras pārskatā ir aplūkotas pieejamās ārstēšanas iespējas, tostarp kognitīvā uzvedības terapija (CBT), farmakoloģiskas iejaukšanās, piemēram, serotonīna atpakaļsaistes inhibitori un antiandrogēnu zāles. Tajā aplūkotas arī alternatīvas psihoterapeitiskās un farmakoloģiskās ārstēšanas iespējas, piemēram, grupu terapija, uz apzinātību balstītas terapijas un ķirurģiska kastrācija. Pārskatā tiek pētīta arī PeD etioloģija, izceļot neiroloģiskās attīstības, ģenētiskos un psihosociālos faktorus. Diskusija par to, kā sociālā stigma ietekmē ārstēšanas pieejamību un kādi ētiski un juridiski izaicinājumi, tostarp obligāta ziņošana un piespiedu ārstēšana, vēl vairāk sarežģī iejaukšanās centienus. Lai gan pašreizējās terapeitiskās pieejas var mazināt simptomus un samazināt bērnu seksuālo nodarījumu (CSO) risku, neviena ārstēšana negarantē pilnīgu efektivitāti. Turpmākajos pētījumos galvenā uzmanība jāpievērš ārstēšanas stratēģiju uzlabošanai, stigmas mazināšanai un profilaktiskās aprūpes pieejamības paplašināšanai personām ar PeD.
Pedophilic Disorder (PeD) is a complex psychiatric condition charachterized by a sustained sexual interest in prepubescent children. This literature review examines available treatment options, including cognitive behavioral therapy (CBT), pharmacological interventions such as serotonin reuptake inhibitors and anti-androgen medications. It also examines alternative psychotherapeutic and pharmacologic treatment options like group therapy, mindfulness based therapies and surgical castration. The review also explores the etiology of PeD, highlighting neurodevelopmental, genetic and psychosocial factors. Discussion of how social stigma affects treatment acceibility and what ethical and legal challenges, including mandatory reporting and involontary treatment further complicate intervention efforts. While current therapeutic approaches can mitigate symptoms and reduce risk of child sexual offence (CSO), no treatment guarantees complete efficacy. Future research should focus on improving treatment strategies, reducing stigma, and exanding access to preventative care for individuals with PeD.
