Abstract
Nosaukums: Urīna nesaturēšana un seksuālā disfunkcija sievietēm pēc dzemdībām: dzemdību norises, seksuālās dzīves un iegurņa pamatnes muskuļu treniņa kontekstā. Aktualitāte: Urīna nesaturēšana ir izplatīta un apgrūtinoša veselības problēma, kas lielākoties skar sievietes. Lai gan urīna nesaturēšana nav dzīvībai bīstams stāvoklis, tā būtiski ietekmē personas dzīves kvalitāti.
Pētījuma mērķis: Izpētīt urīna nesaturēšanu un ar to saistīto seksuālo disfunkciju, kontekstā ar dzemdību norisi, seksuālo dzīvi un iegurņa pamatnes muskuļu treniņa veikšanu sievietēm pēc dzemdībām.
Metodes: Pētījums veidots pēc kvantitatīvas stratēģijas šķērsgriezuma pētījuma dizaina principiem. Interneta vidē tika realizēta vienreizēja aptauja, uzrunājot sievietes 3-24 mēnešus pēc dzemdībām un lūdzot aizpildīt pētījuma instrumentus - pašvērtējuma anketas ICIQ-UI SF, ICIQ-FLUTSsex, FSFI un strukturētu protokolu par vispārējiem datiem, dzemdību norisi un IPM treniņiem. Iegūtie dati tika apkopoti un analizēti izmantojot matemātiskās statistikas metodes.
Rezultāti: UN simptomus pēdējo četru nedēļu laikā bija piedzīvojušas aptuveni puse (53%) no pētījuma dalībniecēm, visbiežak tie tika vērtēti kā vidēji izteikti un norādīja uz SUN, savukārt, pusē gadījumu UN simptomi bija radījuši negatīvu ietekmi uz seksuālo funkciju. Pētījuma dalībniecēm ar UN simtpomiem statistiski ticami biežāk bija vagināls dzemdību veids, šīs nebija pirmās dzemdības un bija biežāk piedzīvotas komplikācijas dzemdību laikā, salīdzinot ar dalībniecēm bez UN simptomiem. Uroloģiskie simptomi (ietverot ne tikai UN) pēdējo četru nedēļu laikā bija negatīvi ietekmējuši seksuālo dzīvi 42,1%iem pētījuma dalībnieču.
Secinājumi: Tika secināts, ka jaunajām māmiņām 3 līdz 24 mēnešu pēc dzemdībām UN ir aktuāla problēma, kas var negatīvi ietekmēt arī seksuālo funkciju, tādejādi mudinot pievērst uzmanību tās risinājumu meklēšanai, lai uzlabotu jauno māmiņu labsajūtu un dzīves kvalitāti. IPM treniņu veicēju zemais īpatsvars starp pētījuma dalībniecēm, kontekstā ar nepietiekošo informētību, liecināja, ka fizioterapijas intervences (t.sk., IPM treniņi) netiek pietiekoši integrētas jauno māmiņu veselības aprūpē, neizmantojot to potenciālos ieguvumus.
Title: Urinary incontinence and sexual dysfunction in postpartum women: in relation to childbirth, sex life and pelvic floor muscle training. Background: Urinary incontinence (UI) is a common and troublesome health problem that mostly affect women. Although urinary incontinence is not a life-threatening condition it has adverse effect on person's quality of life. Objective: The purpose of this study is to explore urinary incontinence and associated sexual dysfunction in context with childbirth, sex life and pelvic floor muscle training. Methods: This reasearch is based on the principles of quantitative research strategy and cross- sectional design. A one-time survey was conducted via social media addressing women 3-24 months postpartum and requesting to participate by filling in a the research tools: self assessment questionnaires ICIQ-UI SF, ICIQ-FLUTSsex, FSFI and a structured protocol which consisted of questions on other relavent data, such as mode of delivery, pelvic floor training etc. The obtained data were collected and analyzed using mathematical statistical methods. Results: Approximately half (53%) of study participants had experienced UI symptoms in the past four weeks. Most commonly UI symptoms refered to stress urinary incontinence (SUI) and were rated as moderate. UI sypmtoms had adverse effects on sexual function in half of cases. Women with UI symptoms had statistically significantly higher incidence of history of vaginal delivery, this was not their first delivery and they were more likely to experience complications during labor compared to continent women. Urological symptoms had adversely affected sexual lives of 42.1% of study participants in the last four weeks. Conclusion: It was concluded that among women 3-24 months after delivery UI is common health problem and it can negatively affect sexual function. These results encourage to look for solutions to improve quality of life among women after delivering a child. The low proportion of women who train PFM in the context of lack of awareness indicated that interventios of physiotherapy (including PFM exercises) are not sufficiently integrated into postpartum health care.
