Bakalaura darba nosaukums: pacientu pieredze par AFO lietošanu pēc SDR operācijas.
Pētījuma mērķis: noskaidrot pacientu, kuriem veikta SDR, pieredzi par AFO lietošanu.
Pētījuma metodes: vairāku gadījumu analīzei izmantotas jauktas datu ieguves metodes. Kvantitatīvo datu ieguvei izmantoti novērtēšanas instrumenti - GMFM-88, MAS, OPUS, kvalitatīvo datu - daļēji strukturēta intervija. Pētījuma izlases veidošanas kritēriji - bērni vecumā no četriem līdz septiņiem gadiem ar diagnosticētu spastisku CT, GMFCS līmenis pirms operācijas II, III vai IV, ir veikta SDR operācija laika posmā no 2022. līdz 2024. gadam, rehabilitācija notiek RC “Poga”, rehabilitācijas procesā tiek izmantots AFO.
Rezultāti: pētījumā piedalījās pieci dalībnieki, kuriem četru līdz septiņu gadu vecumā tika veikta SDR un kuri pēcoperācijas periodā izmantoja AFO. Gandrīz visiem pētījuma dalībniekiem tika novērota mobilitātes uzlabošanās sešus mēnešus pēc SDR. Četriem dalībniekiem GMFM-88 testa rezultāti uzlabojās par 1,82% līdz 8,7%, taču vienam dalībniekam pasliktinājās par 1,44%. Pētījuma dalībniekiem sešus mēnešus pēc SDR tika novērota spasticitātes mazināšanās apakšējo ekstremitāšu muskuļos - pirms operācijas muskuļu tonuss tika novērtēts ar nulle (nav paaugstināta tonusa) līdz pieciem punktiem (stingra fleksija/ekstenzija), taču pēc operācijas tonuss visiem dalībniekiem tika novērtēts ar nulle. Iegūtie dati no OPUS liecināja, ka viszemāko apmierinātību bērnu vecākiem izraisa ilgās gaidīšanas rindas pie tehniskā ortopēda, taču visi vecāki pilnīgi piekrita tam, ka ortozes der labi, kā arī vairumam apgalvojumu par mijiedarbību ar speciālistiem. Galvenās atziņas no daļēji strukturētajām intervijām - bērnu rehabilitācija un ikdiena bez AFO nav iedomājama, ortozes sniedz drošības sajūtu un palielina līdzsvaru, taču ir iespējami AFO uzlabojumi - elastīgāki materiāli, zemāks svars.
Secinājumi: bērniem ar spastisku CT vecumā no četriem līdz septiņiem gadiem SDR var būt efektīva mobilitātes uzlabošanā un spasticitātes mazināšana. AFO lietošana rehabilitācijas procesā un ikdienā ir būtisks elements, kas var uzlabot bērnu aktivitāti.
Bachelor's thesis title: patients' experience of using AFO after SDR surgery. Research objective: to determine the experience of patients who underwent SDR about using AFO. Research methods: a multiple case study design was used, employing mixed methods for data collection. Quantitative data were obtained using assessment tools such as the GMFM-88, MAS and OPUS. Qualitative data were collected through semi-structured interviews. Inclusion criteria for the study sample - children aged four to seven years with a diagnosis of CP, classified as GMFCS levels II, III, or IV prior to surgery, who underwent SDR between 2022 and 2024, are receiving rehabilitation at the RC "Poga," and use AFOs during the rehabilitation process. Results: the study involved five participants who underwent SDR at the age of four to seven and who used AFO in the postoperative period. Almost all study participants observed an improvement in mobility six months after SDR. Four participants improved their GMFM-88 scores by 1,82% to 8,7%, but one participant experienced a decline of 1,44%. Six months after SDR, the study participants experienced a reduction in spasticity in their lower limb muscles - preoperative muscle tone was rated from zero (no increased tone) to five points (rigid flexion/extension), but postoperatively, tone was rated zero in all participants. The data obtained from OPUS showed that the lowest satisfaction among parents of children was caused by long waiting lists for the technical orthopedist, but all parents fully agreed that the orthoses fit well, as well as most statements about interaction with specialists. Key findings from the semi-structured interviews - children's rehabilitation and everyday life without AFOs is unthinkable, orthoses provide a sense of security and increase balance, but improvements to AFOs are possible - more flexible materials, lower weight. Conclusions: in children with spastic CP aged four to seven years, SDR can be effective in improving mobility and reducing spasticity. The use of AFO in the rehabilitation process and in everyday life is an essential element that can improve children's activity.
