Anotācija
Fons: Implantu pārklājēj protezēšanā ir populāra ārstēšanas izvēle pacientiem bez zobiem. Tā nodrošina labāku fiksāciju, stabilitāti un kopējo funkcionalitāti salīdzinājumā ar pilnajām zobu protēzēm. Lai gan zobu protēzes uz implantiem ir atzīta par labāku ārstēšanas veidu, nav vienprātības par vislabāko stiprinājumu metodi protēžu savienošanai ar implantiem. Tas izraisa atšķirības pacientu apmierinātībā ar ārstēšanas rezultātiem.
Mērķi: Šī pētījuma mērķis ir salīdzināt četras populārākus stiprinājumu veidus: tapas, stieņu, lodveida un magnētiskos stiprinājumus. Pētījuma mērķis ir novērtēt šo stiprinājumu veidu efektivitāti, pievēršot uzmanību pacientu apmierinātībai, fiksācijai un apkopes un kopšanas prasībām. Šis pētījums sniegs ārstiem pārskatu, lai palīdzētu pieņemt pamatotus lēmumus par stiprinājumu tipiem, tādējādi uzlabojot pacientu apmierinātību.
Metodes: Tika veikts pašreizējās literatūras sistemātisks pārskats. Iekļautie pētījumi sastāvēja no klīniskajiem pētījumiem, sistemātiskajiem pārskatiem un metaanalīzēm, kas publicētas pēdējo divu desmitgadu laikā. Pētījumi tika atlasīti, balstoties uz to nozīmīgumu implantu pārklājēj protezēšanā, koncentrējoties uz pacientu apmierinātību, fiksāciju un funkcionālitāti.
Rezultāti: Rezultāti liecina, ka, lai gan stieņu stiprinājumi nodrošina izcilu fiksāciju, tie var nebūt piemēroti visām pacientu grupām, jo tiem nepieciešama sarežģīta un laikietilpīga uzturēšana. Tapu un lodīšu stiprinājumi nodrošina pietiekamu pacientu apmierinātību un prasa mazāk apkopes. Magnētiskie stiprinājumi ir piemēroti tikai nišas pacientu grupām, ņemot vērā zemo fiksācijas līmeni un augsto komplikāciju risku. Pacienti parasti dod priekšroku vienkāršām sistēmām, piemēram, tapām un lodītēm, jo tās nodrošina pietiekamu komfortu un ir lietotājam draudzīgas.
Secinājums: Pētījumi rāda, ka stieņu stiprinājumi nodrošina augstāko fiksācijas līmeni, taču to sarežģītība ierobežo popularitāti. Tapu un lodveida stiprinājumi ir visvairāk iecienītā opcija, pateicoties to praktiskumam. Tie nodrošina ideālu līdzsvaru starp fiksāciju, lietošanas ērtumu un uzturēšanu. Magnētiskie stiprinājumi, lai arī piemēroti dažiem pacientiem, paliek mazāk populāra izvēle zemas fiksācijas dēļ. Ārstiem ieteicams ņemt vērā individuālās pacienta vajadzības, lai optimizētu izvēlēto ārstēšanas rezultātu.
Abstract Background: Implant overdentures are a popular treatment choice for patients without teeth. They provide superior retention, stability, and overall functionality compared to conventional complete dentures. Although implant overdentures are a better treatment option, there is no consensus on the best attachment method to connect the overdentures to the implants. This results in differences in patient satisfaction regarding the outcomes of their treatment. Objectives: The objective of this research is to compare the four largest groups of attachments with one another. These groups include stud, bar, ball, and magnet attachments. This research aims to compare the effectiveness of the four largest groups of attachments regarding patient satisfaction, retention and maintenance requirements. This research seeks to equip clinicians with an overview for making informed choices about attachment types. Ultimately, this should contribute to enhancing patient satisfaction. Methods: A systematic review of the current literature was conducted. The included literature consisted of: clinical trials, systematic reviews, and meta-analyses published in the past two decades. Studies were chosen for their significance to implant overdentures, focusing on patient satisfaction, retention, and functional performance outcomes. Results: The results suggest that although a bar attachment offers superior retention, it may not suit all patient groups because of its complex and high maintenance requirements. Stud and ball attachments were found to provide adequate patient satisfaction while requiring less maintenance. Magnetic attachments are suitable for only a niche group of patients due to their low retention factor and high risk of complications. Patients generally tend to favour straightforward systems like studs and balls, as they provide sufficient comfort and are user-friendly. Conclusion: Research shows bar attachments offer the highest amount of retention, but their complexity limits their popularity. Stud and ball attachments are the most favoured option because of their practicality. They strike a perfect balance between retention, user-friendliness, and maintenance. Although suitable for some patients, magnetic attachments remain the least popular choice due to their low retention. Clinicians are advised to consider individual patient needs to optimise the perceived outcome of the treatment.
