Koncentrējoties uz zoba dzīvotspējas saglabāšanu un kariesa bojājumu progresēšanas novēršanu, dziļā kariesa ārstēšana ir būtisks aspekts restauratīvajā zobārstniecībā. Šajā darbā tika analizētas un salīdzinātas divas plaši lietotas dziļā kariesa ārstēšanas metodes: totāla kariesa ekskavācija un selektīva kariesa ekskavācija. Totāla kariesa ekskavācija paredz visu kariesa audu izņemšanu vienas vizītes laikā, un tas palielina pulpas ekspozīcijas un iespējamo komplikāciju, piemēram, pulpas nekrozes, risku. Savukārt selektīva kariesa ekskavācija ir konservatīvāka metode, kas dod laikus terciārā dentīna izveidei pirms pilnīgas kariesa audu izņemšanas nākamajā ārstēšanas seansā. Šī metode ir saistīta ar lielāku iespēju saglabāt pulpas dzīvotspēju un samazināt pulpas traumatiskas atvēršanas risku. Pētījumā tika novērtēta abu pieeju efektivitāte, analizējot to ietekmi uz pulpas audu dzīvotspēju un zoba ilgtermiņa prognozi. Esošo pētījumu pārskats, tostarp Bjørndal et al. (2010), Khokhar et al. (2018) un Oz et al. (2019), norāda uz atšķirībām starp abām metodēm. Lai gan dažos pētījumos, veicot radioloģisko diagnostiku, totāla kariesa ekskavācija uzrādījusi labākus klīniskos rezultātus, selektīva kariesa izņemšana pārliecinoši demonstrē zemāku pulpas traumatiskas atvēršanas risku un labāku dzīvotspēju. Secinājumi liecina, ka selektīvā kariesa izņemšana metode ir vēlamā pieeja dziļā kariesa ārstēšanā.
Managing deep caries is a crucial aspect of restorative dentistry, focusing on preserving tooth vitality and preventing the progression of carious lesions. This thesis explores and compares two widely used techniques for deep caries treatment: total excavation and stepwise excavation. Total excavation involves the complete removal of carious tissue in a single visit, which increases the risk of pulp exposure and potential complications, such as pulp necrosis. In contrast, stepwise excavation is a more conservative two-step process that allows time for the pulp to repair itself before completing the caries removal. This technique is associated with a higher likelihood of preserving pulp vitality and minimizing the risk of pulp exposure. This study evaluates the effectiveness of both approaches by examining their impact on pulp exposure, pulp vitality, and the long-term success of the tooth. A review of existing research, including studies by Bjørndal et al. (2010), Khokhar et al. (2018), and Oz et al. (2019), highlights the differences between the two methods. While total excavation has shown higher radiographic success in some studies, the stepwise technique consistently demonstrates lower rates of pulp exposure and better preservation of pulp vitality. The findings suggest that stepwise excavation is the preferred approach for managing deep caries, offering a more conservative treatment option with fewer risks and better long-term outcomes for tooth vitality.
