Juvenilais idiopātiskais artrīts (JIA) ir biežākā reimatiskā saslimšana bērniem. Uveīts ir viena no izplatītākajām JIA komplikācijām. Tas var novest pie smagas redzes pasliktināšanās, vai pat akluma. Uveīta agrīnas diagnostikas sekmēšana samazinātu tālāku okulāro komplikāciju risku. Tādēļ ir jānovērtē uveīta sastopamības biežums, uveītu veicinošie faktori un izmantotā terapija. Retrospektīvais pētījums veikts, apkopojot Bērnu Klīniskās Universitātes Slimnīcas (BKUS) informāciju sistēmu programmā „Andromeda” pieejamās pacientu slimības vēstures. Pētījumā iekļauti pacienti, kuri laika periodā no 2017. gada 1. septembra līdz 2022. gada 1. septembrim vismaz vienreiz stacionēti BKUS, saistībā ar JIA diagnostiku un/vai terapiju. Pa-cientus iedalīja divās grupās – JIA pacienti bez uveīta un JIA pacienti ar uveītu. Apkopotajos datos ietilpa informācija par pacientu vecumu, dzimumu, JIA apakštipu, laiku starp ar JIA sais-tītu muskuloskeletālu simptomu parādīšanos un JIA diagnozes noteikšanu, asinsainas vērtībām, immunoģenētiskajām atradēm, iekaisuma marķieriem, ģimenes anamnēzi, komorbiditātēm un pielietoto ārstēšanu. Papildus tam ieguva informāciju par akūta vai hroniska uveīta sastopamī-bas biežumu, uveīta noteikšanas lauku, uveīta komplikācijām un uveīta remisiju. Pētījumā iekļāva 546 JIA pacientus, no tiem 5,1% (n=28) bija noteikts uveīts. No uveīta pacientiem 64,3% bija meitenes. Uveīta pacientiem vidējais vecums JIA noteikšanas brīdī bija 5,5 gadi, savukārt vidējais vecums uveīta noteikšanas brīdī bija 7,4 gadi. Uveīts bija noteikts oligoartrīta (53,6%), seronegatīva poliartrīta un nediferencēta artrīta pacientiem. Starp pacien-tiem, kuru slimības vēsturēs bija informācija par immunoģenētiskajām atradēm JIA noteikšanas laikā, 73,1% bija antinukleārāo antivielu (ANA) pozitīvi, 89,3% - HLA-B27 negatīvi un 100% bija reimatoīdā faktora (RF) negatīvi. Trombocitoze JIA noteikšanas laikā bija novērojama 30% uveīta pacientu. Pozitīva ģimenes vēsture bija pieejama mazāk, nekā cetrtaļai JIA pacientu. Uveīta pacientiem varēja novērot 17 dažādas okulārās komplikācijas. Abās grupās populārākais bioloģiskais slimību modificējošais antireimatiskais medikaments (SML) bija adalimumabs – to lietoja 13,7% JIA pacientu bez uveīta un 39,3% ar uveītu. JIA izraisīts uveīts biežāk tika novērots meitenēm, kā arī ologoartrītiska JIA pacientiem, RF un HLA-B27 negatīviem, un ANA pozitīviem pacientiem, kā arī tiem pacientiem, kuriem JIA diagnoze tika noteikta agrākā vecumā. Gandrīz trešdaļai pacientu uveīts bija noteikts jau pirms JIA. Visizplatītākā uveīta komplikācija bija mugurējās sinehijas, kas skāra pusi uveīta pacientu. Ķīmisko SML neefektivitāte bija novērota gandrīz pusei pacientu, kuriem noteica uveītu. Uveīta remisija bija novērota vairāk, nekā pusei uveīta pacientu.
Juvenile idiopathic arthritis (JIA) is the most common rheumatic disease in children. Uveitis is one of it’s the most common complications. It can result in severe vision impairment, or even blindness. Early diagnosis of uveitis decreases the risk of further ocular complications. To achieve that, we must analyse uveitis frequency, contributing factors and JIA therapy. Retrospective study was performed by gathering data from patient’s histories, available in Children’s clinical university hospital online database “Andromeda”. The study included all patients that were hospitalized at least once, to receive treatment for JIA, in the last five years (the 1st of September 2017 – 1st of September 2022). The patients were divided into two groups - with and without uveitis. Relevant data included information about patients age, gender, JIA subtype, time between the start of musculoskeletal symptoms and the diagnosis of JIA, blood count values, immunogenetic findings, inflammation markers, family history, comorbidities, and therapy. In addition, data were gathered regarding the frequency of uveitis, time of uveitis diagnosis, uveitis complications and uveitis remission. The study included 546 JIA patients, of them 5,1% (n=28) were diagnosed with uveitis. Girls comprised 64% of uveitis patients. Uveitis patients had a median age of 5.5 years at the time of JIA diagnosis, while the median age at the time of uveitis determination was 7.4 years. Uveitis was detected in oligoarthritis (53.6%), seronegative polyarthritis (42,9%) and non-diffe-rentiated arthritis (3,6%) patients. Among patients with histories of immunogenetic findings at the time of JIA diagnosis, 73.1% were anti-nuclear antibody (ANA), 89.3% were HLA-B27 negative and 100% were rheumatoid factor (RF) negative. Thrombocytosis was observed in 30% of uveitis patients at the time of JIA diagnosis. Positive family history was found in less than one fourth of JIA patients. There were 17 different ocular complications found in uveitis patients. In both groups, adalimumab was the most popular biological disease modifying antir-heumatic drug (DMARD), used in 13.7% of uveitis-free JIA patients and in 39.3% with uveitis. Uveitis caused by JIA was more commonly seen in girls, as well as in ologoarthritic JIA patients, RF and HLA-B27-negative, and ANA positive patients, as well as in patients diag-nosed with JIA at an earlier age. Almost a third of patients had uveitis before JIA. The most common complication of uveitis was posterior synechia, which affected half of uveitis patients. The resistance against chemical DMARDs was observed in almost half of patients treated with uveitis. Uveitis remission was seen in more than half of uveitis patients.
