Scapholunate (SL) saite, visbiežāk traumētā plaukstas locītavas saite, ir būtiska karpālā stabilitātes uzturēšanai, savienojot proksimālos un distālos plaukstas kaulus. SL saišu bojājumi var izraisīt sāpes, ierobežotu kustību diapazonu (ROM) un spēka samazināšanos, tādējādi negatīvi ietekmējot dzīves kvalitāti. Ja šādas traumas neārstē, tās var izraisīt progresējošu Scapholunate kolapsu (SLAC) vai deģeneratīva artrīta attīstību. Neskatoties uz SL saišu traumu izplatību, joprojām nav vienprātības par optimālo ķirurģisko pieeju - artroskopisku vai atklātu rekonstrukciju.
Šī literatūras apskata mērķis ir novērtēt un salīdzināt šo divu ķirurģisko metožu pēcoperācijas rezultātus, koncentrējoties uz tādiem galvenajiem rādītājiem kā ROM, satvēriena stiprums, sāpju rādītāji, SL spraugas samazināšana un komplikāciju biežums, lai piedāvātu standartizētu ārstēšanas protokolu. Rezultāti liecina, ka artroskopiskās iejaukšanās liecina par labākiem rezultātiem sāpju mazināšanas, ROM, saķeres spēka un samazinātu komplikāciju biežuma ziņā, salīdzinot ar atvērtu SL saišu rekonstrukciju. Tomēr ir nepieciešami turpmāki ilgtermiņa pētījumi, lai pilnībā novērtētu abu pieeju ilgstošu efektivitāti.
The Scapholunate (SL) ligament, the most frequently injured ligament in the wrist, is essential for maintaining carpal stability by linking the proximal and distal carpal bones. Damage to the SL ligament can result in pain, restricted range of motion (ROM), and diminished strength, thereby negatively impacting quality of life. If left untreated, such injuries may lead to Scapholunate advanced collapse (SLAC) or the development of degenerative arthritis. Despite the prevalence of SL ligament injuries, there remains no consensus on the optimal surgical approach - arthroscopic or open reconstruction. This literature review seeks to evaluate and compare the postoperative outcomes of these two surgical techniques, focusing on key indicators such as ROM, grip strength, pain scores, SL gap reduction, and complication rates, with the aim of proposing a standardized treatment protocol. Findings indicate that arthroscopic interventions demonstrate superior outcomes in terms of pain relief, ROM, grip strength, and reduced complication rates compared to open SL ligament reconstruction. Nonetheless, further long-term studies are necessary to fully assess the sustained efficacy of both approaches.
