Meta-analysis of Preventive Analgesic Effect of Systemic Magnesium Sulphate on Postoperative Pain

ABSTRAKTS Pamats un mērķi: Pēcoperācijas sāpes ir ļoti izplatīta komplikācija pēc operācijas. Pētījumi ir parādījuši, ka intravenoza magnija sulfāta (turpmāk minētā kā magnija) ievadīšana ir pozitīvi ietekmējusi sāpju mazināšanu un opioīdu lietošanu. Tomēr operācijas laikā to joprojām regulāri neizmanto. Šī metaanalīze galvenokārt novērtēs magnija profilaktisko pretsāpju iedarbību, kā arī sekundāri pētīs magnija ievadīšanas optimālo laiku un devu, anestēzijas ilgumu un hipotensijas risku. Metodes: Tika izmantotas elektroniskās datu bāzes ScienceDirect, EBSCO host un PubMed, lai atrastu jaunākos pētījumus no 2016. līdz 2019. gadam. Kopumā tika pārbaudīti un pārskatīti 31 pētījumi. Seši pētījumi, kuros piedalījās 374 pacienti, tika novērtēti pēc iekļaušanas kritērijiem un pārskatīti turpmākai metaanalīzei. Šajos pētījumos tika pētīti sāpju rezultāti, salīdzinot sāpju rādītājus 0-2, 6 un 24 stundas pēc operācijas starp testa grupām (saņemot magniju) un kontroles grupām. Paralēli tika iegūta visefektīvākā magnija ievadīšanas deva un laiks, kā arī intraoperatīvās hipotensijas biežums. Statistiskā nozīmība tika uzskatīta ar p <0,05. Rezultāti: Pārskatīto pētījumu vizuālās analogās skalas (VAS) sāpju rādītāju salīdzinājums pierādīja statistiski nozīmīgu pēcoperācijas sāpju uzlabošanos visos analizētajos laika punktos: 0–2 stundas pēc operācijas vidējais VAS sāpju rādītājs kontrolgrupā salīdzinājumā ar testa grupu attiecībā pret uz parauga grupas lielumu bija 5,31 pret 4,73; p <0,0005; pēc 6 stundām bija 4,47 pret 3,82; p <0,0005; un pēc 24 stundām 3,3 pret 2,77; p <0,0005. Optimālā magnija ievadīšanas deva ir 20-40 mg / kg pirmās operācijas stundas laikā un kopumā aptuveni 25-50 mg / kg. Pēc indukcijas magnija ievadīšana jāsāk ar 20-30 mg / kg bolusu un jāturpina kā infūzija līdz operācijas beigām. Lielākas kopējās magnija devas ievērojami neuzlaboja pēcoperācijas sāpju rādītāju. Magnijs neietekmēja anestēzijas ilgumu un būtiski nepalielināja hipotensijas risku intraoperatīvi. Secinājumi: Magnijam ir ievērojama profilaktiska pretsāpju iedarbība, ja to lieto sistemātiski, un tā iedarbība ir saistīta ar devu un ievadīšanas laiku. Kopumā apmēram 25-50 mg / kg magnija deva, ko ievada bolus veidā un turpina līdz operācijas beigām, ir pietiekama, lai mazinātu pēcoperācijas sāpes, nepalielinot intraoperatīvās hipotensijas risku.
ABSTRACT Background and objectives: Postoperative pain is a very common complication after surgery. Studies have shown that intravenous magnesium sulphate (in the following mentioned as magnesium) administration has shown positive effects on pain relief and opioid consumption. However, it is still not routinely used during surgery. This meta-analysis will primarily evaluate the preventive analgesic effects of magnesium, and secondarily investigate the optimal time and dosage of magnesium administration, duration of anaesthesia and the risk of hypotension. Methods: The electronical databases ScienceDirect, EBSCO host and PubMed were searched including recent studies from 2016 to 2019. A total of 31 studies were screened and reviewed. Six trials including 374 patients were evaluated according to the inclusion criteria and reviewed for the further meta-analysis. These trials investigated pain outcomes comparing pain scores at 0-2, 6 and 24 hours postoperatively between test groups (receiving magnesium) and control groups. Parallelly, the most effective dosage and time of magnesium administration and incidence of intra-operative hypotension was obtained. Statistical significance was considered with p <0.05. Results: The comparison of visual analogue scale (VAS) pain score of the reviewed studies proved a statistically significant improvement of postoperative pain in all analysed time points: 0-2 hours after the surgery the mean VAS pain score in control vs test group in relation to the sample size was 5.31 vs 4.73; p < 0.0005; after 6 hours it was 4.47 vs 3.82; p < 0.0005; and after 24 hours 3.3 vs 2.77; p < 0.0005. The optimal dosage of magnesium administration was identified to be 20-40 mg/kg during the first hour of surgery and a total of approximately 25-50 mg/kg. Magnesium administration should be initiated as a bolus of 20-30 mg/kg after induction and continued as an infusion until the end of surgery. Higher dosages of total magnesium administration did not improve the postoperative VAS pain score significantly. Magnesium did not influence the duration of general anaesthesia and did not significantly increase the risk of hypotension intraoperatively. Conclusion: Magnesium has significant preventive analgesic effects when given systemically and is related to the dosage and time of administration. A total of approximately 25-50 mg/kg magnesium, administered as a bolus and continued until the end of surgery, is sufficient to reduce the postoperative pain without increasing the risk of intraoperative hypotension.
