Abstract
Zobu autotransplantācija ir zoba pārvietošana no vienas alveolas uz citu vienam cilvēkam. Šī darba mērķis bija izpētīt un analizēt trešo molāru autotransplantāciju indikācijas, operatīvos etapus un veiksmes rādītājus pēc literatūrā pieejamiem datiem. Tika izvēlēti 28 raksti, no kuriem var secināt, ka vislabāk transplantācijai izmantot zobus ar sakni III vai IV attīstības stadijā un veikt transplantāciju tūlītēji pēc neatjaunojamā zoba ekstrakcijas, tomēr var izveidot arī jaunu alveolu sadzijušajā kaulā. Operācijas laikā var ievērojami samazināt transplantāta ekstraorālo laiku, izmantojot zobu 3D replikas. Svarīgi ir fleksibli fiksēt zobu jaunajā alveolā, lai samazināt ankilozes risku. Zobiem ar pabeigtu saknes formēšanos parasti būs nepieciešama endodontiskā ārstēšana pēc iniciālās dzīšanas. Literatūrā pieejamie dati parāda, ka trešo molāru autotransplantācija var būt tikpat efektīva un ievērojami lētāka metode neatjaunojamo zobu aizstāšanai, kā konvencionālie zobu implanti, kā arī tā var tikt izmantota pediatrijas pacientiem. Uz doto brīdi ir pieejams nepietiekami daudz ilgtermiņa literatūras par trešo molāru autotransplantāciju, lai paredzami novērtēt ārstēšanas gala iznākumu.
Tooth autotransplantation is the movement of a tooth from one site to another in the same person. The aim of this paper was to research and analyze the indications, operating stages and success rates of third molar autotransplantation based on the data of current literature. A total of 28 studies were included from which it is evident that the most successful are immature third molars at root development stages III and IV which are transplanted to a fresh extraction site immediately after extraction of the non-retainable tooth, although it is also possible to create a new socket surgically in an already healed bone. During the operation the use of 3D printed donor tooth replica can substantially lower the extra-oral time of the donor tooth. It is necessary to use flexible splinting methods to minimize the risk of ankylosis. Teeth with complete root formation usually require endodontic treatment after initial healing period. The findings from the current literature suggest that third molar autotransplantation could be as successful and much more cost-effective treating method as conventional dental implants, also it could be used for pediatric patients. However, the long-term literature relating to the third molar autotransplantation is limited, so further investigation with longer follow-up periods is required.
