Išēmisks insults ir otrais biežākais mirstības cēlonis un trešais biežākais darbnespējas cēlonis pasaulē. Šajā literatūras apskatā tiek aplūkota neaterosklerotisku neiekaisīgu arteriopātiju saistība ar išēmisku insultu, īpašu uzmanību pievēršot tam, kā neaterosklerotiskas neiekaisīgas arteriopātijas ietekmē išēmiska insulta pacientu ārstēšanu un prognozi. Apskatā tika iekļautas tādas neaterosklerotiskas neiekaisīgas arteriopātijas kā dolihoektāzija, Vilīzija loka anatomiskās variācijas, carotid web, neplīsušās cervikālu un cerebrālu artēriju aneirismas, cervikālu un cerebrālu artēriju disekcijas un Moyamoya slimība. Publikācijas tika atlasītas, izmantojot brīvpieejas datubāzes “PubMed” un Rīgas Stradiņa universitātes abonēto zinātnisku e-žurnālu meklēšanas rīkus. Tika konstatēts, ka neaterosklerotiskas neiekaisīgas arteriopātijas ir insulta pacientiem ir sastopamas biežāk nekā vispārējā populācijā. Nepilns Vilīzija loks un carotid web saistās ar sliktāku pacientu prognozi, un neplīsušām aneirismām biežāk rodas ar ārstēšanu saistītas komplikācijas. Carotid web un artēriju disekcijas var potenciāli kalpot par išēmiska insulta etiopatoģenētisku faktoru, un tiem ir pieejama ķirurģiska ārstēšana. Esošie akūta išēmiska insulta algoritmi ir pietiekami droši pacientiem ar neaterosklerotiskām neiekaisīgām arteriopātijām, lai diagnostiku varētu atlikt un veikt pēc neatliekamas ārstēšanas. Nepieciešami turpmāki prospektīvi, randomizēti pētījumu, kas apstiprinātu neaterosklerotisku neiekaisīgu arteriopātiju ietekmi uz ārstēšanu un pacienta iznākumu.
Ischemic stroke is the second leading cause of mortality and the third leading cause of disability worldwide. In this literature review, the relationship between non-atherosclerotic non-inflammatory arteriopathies and ischemic stroke was examined, with special emphasis on the treatment and prognosis of such patients. Dolichoectasia, anatomical variations of the Circle of Willis, carotid webs, unruptured arterial aneurysms, cervical and cerebral arterial dissections and Moyamoya disease were reviewed. Publications were selected by performing a search in the open access database “PubMed” and scientific e-journals Riga Stradins university has subscription for. It was determined that non-atherosclerotic non-inflammatory arteriopathies are more common in ischemic stroke patients than in the general population. Incomplete Circle of Willis and carotid web were predictors for an unfavorable patient prognosis, and unruptured aneurysms were associated with more frequent treatment-related complications. Carotid web and arterial dissection are potential etiopathogenetic ischemic stroke causes, and surgical treatment is possible for these patients. Current acute ischemic stroke algorithms are safe enough in patients with non-atherosclerotic non-inflammatory arteriopathies, meaning the diagnostics can be performed after the emergency treatment. Further prospective, randomized studies are necessary to confirm non-atherosclerotic non-inflammatory arteriopathies’ influence of the treatment and outcome of the ischemic stroke patients.
