Pamatinformācija: Akūts apendicīts (AA) ir viena no visbiežāk diagnosticētajām ķirurģiskajām slimībām pasaulē. Tomēr tā diagnostika ir sarežģīta, jo īpaši pediatrijā. Lai gan pēdējās desmitgadēs ir gūti panākumi tās diagnostikā un ārstēšanā, AA joprojām ir viena no visbiežāk nepareizi diagnosticētajām neatliekamajām ķirurģiskajām slimībām. Pareizas un savlaicīgas diagnozes nodrošināšana ir svarīga, lai novērstu komplikāciju rašanos, kā arī nevajadzīgu operāciju skaitu. Pašreizējā AA diagnostika balstās uz anamnēzes datiem, fizikālo izmeklēšanu, laboratoriskajiem testiem un radioloģiskajiem izmeklējumiem. AA diagnostikas kontekstā ir parādījušies jauni biomarķieri. Daži no biomarķieriem, kuriem pēdējos gados pievērsta īpaša uzmanība, ir interleikīns-6 (IL-6), ar neitrofilu želatināzi saistītais lipokalīns (NGAL) un ar leicīnu bagātais alfa-2 glikoproteīns 1 (LRG-1). Mērķis: pārskatīt un iepazīstināt ar seruma un urīna biomarķieru - īpaši IL-6, NGAL un LRG-1 - diagnostikas iespējām akūta apendicīta gadījumā bērniem (PAA). Hipotēze: Seruma un urīna biomarķieri IL6, NGAL un LRG-1 var uzlabot PAA diagnostikas precizitāti un papildus palīdzēt atšķirt komplicētu un nekomplicētu slimību. Metodes: Šajā literatūras apskatā tika pārskatītas septiņas zinātniskās publikācijas, lai novērtētu seruma un urīna IL-6, NGAL un LRG-1 diagnostikas potenciālu AA diagnosticēšanai pediatriskiem pacientiem. Zinātniskie pētījumi publicēti laikā no 2018. līdz 2023. gadam. Secinājumi: Serumā NGAL, LRG-1 un LRG-1 urīnā var kalpot kā vērtīgi PAA diagnostikas marķieri. Seruma biomarķieris IL-6 joprojām ir nespecifisks AA diagnozes apstiprināšanai, bet to var izmantot saslimšanas smaguma novērtēšanai, lai nošķirtu CAA no NCAA. Biomarķieri ir jāizmanto piesardzīgi, un ir ieteicams tos iekļaut multimodālā diagnostikas pieejā kopā ar tādām tradicionālām metodēm kā klīniskais novērtējums, laboratoriskie izmeklējumi un attēldagnostika. Šie secinājumi būtu jāapsver to priekšrocību un ierobežojumu kontekstā, lai virzītu turpmākos pētījumus un potenciālo klīnisko pielietojumu.
Background: Acute appendicitis (AA) is one of the most frequently diagnosed surgical diseases worldwide. However, its diagnosis is challenging, especially in pediatrics. Although advances have been made in recent decades regarding its diagnosis and treatment, AA remains one of the most frequently misdiagnosed surgical emergencies. Ensuring correct and timely diagnosis is important to prevent the development of complications, as well as prevent the rate of unnecessary surgeries. The current diagnosis of AA is based on anamnestic evidence, physical examination, laboratory tests, and radiological investigations. Novel biomarkers have emerged in the context of the diagnosis of AA. Some of the biomarkers that have gained more attention in recent years are interleukin-6 (IL-6), neutrophil gelatinase-associated lipocalin (NGAL), and leucine-rich alpha-2 glycoprotein 1 (LRG-1). Aim: Review and present the diagnostic possibilities of serum and urine biomarkers IL-6, NGAL, and LRG-1, for the diagnosis of pediatric acute appendicitis (PAA). Hypothesis: Serum and urine biomarkers IL-6, NGAL, and LRG-1 can improve the diagnostic accuracy of PAA, and, in addition, help differentiate between complicated and non-complicated disease. Methods: In this review of the literature, seven scientific publications were reviewed to assess the diagnostic potential of serum and urine IL-6, NGAL, and LRG-1 for diagnosing AA in pediatric patients. The scientific studies were published between 2018 and 2023. Conclusion: Serum NGAL, LRG-1 and urinary LRG-1 have the potential to serve as valuable diagnostic markers for PAA. The serum biomarker IL-6 remains unspecific for AA diagnosis confirmation, but it can be used for severity assessment to differentiate between CAA and NCAA. The biomarkers should be used with precaution, and it is advisable to intergrate them into a multimodal diagnostic approach, in conjunction with conventional methods like clinical assessment, laboratory tests, and medical imaging. These findings should be considered in the context of their strengths and limitations to guide future research and potential clinical applications.
