Impact of Prenatal Detection of the Transposition of the Great Arteries on Postnatal Outcome

Pateicoties mūsdienu medicīnas tehnoloģijām ir iespējams veikt prenatālo diagnostiku, ļaujot agrīni identificēt tādas iedzimtas sirds slimības kā maģistrālo asinsvadu transpozīciju. (MAT). Literatūras apskata mērķis ir izpētīt MAT prenatālās diagnostikas ietekmi uz pēcdzemdību iznākumu, galvenokārt novērtējot ietekmi uz mirstību, saslimstību un dzemdību organizēšanu terciārās aprūpes centros. Pētījumā tika izmantota sistemātiska meklēšana PubMed un Primo datubāzēs, lai identificētu atbilstošus pētnieciskos rakstus, aptverot plašu līdz šim publicēto pētījumu klāstu. Darba rezultāti atklāja MAT prenatālās diagnostikas ietekmi uz pēcdzemdību iznākumu. Darbā atlasīto pētījumu vispusīga izpēte liecina par pozitīvu tendenci: labāks stāvoklis pēc dzimšanas jaundzimušajiem ar MAT, kuriem patoloģija tika diagnosticēta prenatāli, salīdzinājumā ar tiem, kuriem tā nebija. Literatūras apskata rezultāti neliecina par samazinātu mirstību un īsāku uzturēšanos slimnīcā, taču liecina par retākām komplikācijām un efektīvāku ārstēšanu plānojot dzemdībās terciārās aprūpes centros pacientiem, kuriem MAT diagnosticēta prenatāli. Kopumā pacientiem, kuriem MAT diagnosticēta prenatāli, ir labāka prognoze salīdzinājumā ar pacientiem, kuriem MAT diagnosticēta postnatāli. Analizējot saistību starp publikācijas gadu un prenatālās diagnostikas procentuālo īpatsvaru, tika konstatēta pozitīva korelācija : jo jaunāks publikācijas gads, jo augstāks MAT prenatālas diagnostikas procents. Noslēgumā jāsecina, ka šis literatūras apskats apstiprina faktu, ka maģistrālo asinsvadu transpozīcijas prenatāla diagnostika pozitīvi ietekmē pēcdzemdību rezultātus. MAT prenatālās diagnostikas procentuālais īpatsvars joprojām ir salīdzinoši zems, tāpēc ir nepieciešami turpmāki pētījumi, lai izpētītu dažādus aspektus, kas ietekmē pēcdzemdību iznākumu bērniem ar MAT. The advances in medical techniques over the years have revolutionized prenatal diagnosis, making possible the early identification of congenital heart diseases such as transposition of the great arteries (TGA). The literature review aims to investigate the increase in prevalence of prenatal diagnosis of TGA in the last decades and detect the impact of prenatal diagnosis of TGA on postnatal outcomes, mainly focusing on mortality, timeliness of treatment and efficacy, length of stay in hospital, and delivery in tertiary care centers. To accomplish this, systematic searches on PubMed and Primo databases were performed to identify relevant research articles, encompassing a wide range of studies published up to date. The analysis revealed an overall positive impact of the prenatal diagnosis of TGA on postnatal outcomes. After a comprehensive examination of the studies selected for the research, a positive trend towards improved postnatal outcomes for infants with TGA who received prenatal diagnosis compared to those who did not was observed. Although the data do not provide conclusive evidence of reduced mortality rates or shorter hospital stays, they do support the provision of adequate and prompt treatment for patients diagnosed prenatally compared to those diagnosed postnatally. Overall, patients diagnosed prenatally with TGA demonstrated a better prognosis compared to patients diagnosed postnatally. No statistically significant results were obtained analyzing the relationship between the year of publication and the percentage of prenatal diagnosis, but a weak positive correlation was found between the later study in time and the higher prenatal diagnosis percentage. In conclusion, this literature review supports the fact that prenatal diagnosis of transposition of the great arteries has a positive impact on postnatal outcomes. The percentage of prenatal diagnosis of TGA still remains relatively low and thus further research is warranted to explore different aspects of the impact on postnatal outcomes of patients diagnosed with TGA.
