Abstract
Tēmas nosaukums: Aprūpes problēmas un to risinājumi pacientiem ar abdominālo vakuuma asistēto noslēgšanas sistēmu.
Pētījuma Mērķis: Noskaidrot, kādas ir aprūpes problēmas un to risinājumi pacientiem ar abdominālo VAC sistēmu intensīvās terapijas nodaļās.
Darba tēma tika izvēlēta, jo tēma ir maz apskatīta, un Latvijā par šo tēmu informācijas praktiski nav, līdz ar to diplomdarba ietvaros tika izvirzīti divi pētāmie jautājumi:
1. Kādas ir aprūpes problēmas pacientiem ar abdominālo VAC sistēmu intensīvās terapijas nodaļā?
2. Kādi ir aprūpes problēmu risinājumi pacientiem ar abdominālo VAC sistēmu intensīvās terapijas nodaļā?
Darba aprakstošajā daļā tika apkopota informācija par to, kas ir abdominālā VAC sistēma (arī terapija), kādas ir indikācijas, kontrindikācijas, komplikācijas, kā arī ar to saistītās aprūpes problēmas un potenciālie risinājumi.
Darba pētniecības daļas ietvaros tika izveidota intervija ar 12 atvērtā tipa jautājumiem, uz kuriem atbildēja 7 respondenti, kuri strādā intensīvās terapijas nodaļā. Jautājumi tika veidoti tā, lai iegūtu vispārēju priekštatu par respondentu viedokli, par abdominālo VAC sistēmu un atbildētu uz pētījumā izvirzītajiem jautājumiem, radot priekštatu par aprūpes problēmām šiem pacientiem un iespējamiem risinājumiem.
Iegūtie intervijas dati tika apstrādāti un sadalīti tabulās pēc kontentanalīzes metodes, pēc kā seko apraksts.
Veicot rezultātu apkopošanu, pēc respondentu atbildēm var spriest, ka māsām ir vispārējs priekštats par VAC sistēmu un tās darbības principiem, kā arī to, ka pacientiem ar abdominālo VAC ir mazāk aprūpes problēmu nekā līdzīgiem pacientiem ar pasīvām drenām vai nenoslēgtu vēderu. Kā biežākās aprūpes problēmas tiek izvirzītas: sāpes, pozicionēšanas grūtības, netiešā veidā tiek norādīts arī uz nepietiekamu enterālo barības vielu uzņemšanu, kā arī tiek apskatītas tādas problēmas kā motivācijas trūkums, pierašana pie narkotiskajiem atsāpināšanas līdzekļiem un ādas bojājumiem.
Thesis of the study: Nursing care problems and their solutions in patient with Vacuum Assisted Closure (VAC) system. The aim of the study: to find out what are the care problems and their solutions for patients with abdominal vacuum assisted closure system in intensive care unit (ICU). The topic of this thesis was chosen, as it is not well enough covered and there is practically no information on this topic in Latvia, thus two research questions were raised within this study: 1. What are the abdominal vacuum closure patient care problems in intensive care unit? 2. What are possible solutions for these care problems in intensive care unit? The descriptive part of the paper summarized the information about the abdominal VAC system (including therapy), indications and contraindications of its use, complications as well as related care problems and possible solutions for them. An interview with 12 open-ended questions was created for the practical part of this research, which were answered by 7 nurses working in the intensive care unit. Questions were designed to get a general idea of the nurses’ opinion about the abdominal VAC system and to answer the questions raised in this study, giving an idea of the care problems for these patients and possible solutions. Obtained data were processed and divided into tables according to the content analysis method, followed by a description. According to the answers of the respondents (nurses), it can be concluded that the nurses have a general idea of the VAC system and its operating principles in ICU setting. Nurses have also noted that patients with abdominal VAC have fewer care problems than similar patients with passive drains or temporarily closed abdomen without the VAC system. The most common care problems in patients with VAC system are pain and positioning difficulties. Also, respondents indirectly point to insufficient intake of enteral nutrients, as well as lack of motivation from patients, addiction to narcotic analgesics and skin lesions.
