Primārās aprūpes medicīnas darbinieki, īpaši ārsti, vispārējās veselības aprūpes ietvaros var ievērojami uzlabot savu pacientu mutes veselību (Tadin & Dzaja, 2023). Ģimenes ārsti, pediatri biežāk redz jaundzimušos un bērnus nekā zobārsti, tāpēc, lielākoties, viņiem ir pirmais kontakts ar bērnu un vecākiem, un iespēja izglītot par bērna mutes veselību, mutes slimību profilaksi un ārstēšanu (Tadin & Dzaja, 2023). Darba mērķi bija izveidot un validēt aptaujas anketu “Ģimenes ārstu zināšanas un iesaiste bērnu mutes veselības veicināšanā’’ un izpētīt ģimenes ārstu zināšanas un iesaisti bērnu mutes veselības veicināšanā. Latvijā jaunākā informācija par kariesa izplatību un intensitāti 12 gadus veciem bērniem, tika apkopota 2021.gadā un rezultāti parādīja, ka K1PEz= 98,5% (K1 apzīmē emaljas kariesu bez kavitātes; K-kariess, P-plombēti zobi, E-ekstrahēti zobi), K5PEz= 71,9% (K5 apzīmē kavitāti dentīnā) (Maldupa et al., 2021). Latvijā bērnu kariesa pieredze vēl joprojām ir augsta. Neārstēts kariess bērnam jebkurā vecumā vai agrīns bērnības kariess negatīvi ietekmē bērna pašsajūtu un veselību - ķermeņa svaru (uztura problēmas, grūtības ēst, košļāt), izaugsmi, attīstību, komunikāciju, atzīmes skolā (Tadin & Dzaja, 2023; Uribe et al., 2021). Ģimenes ārstiem un citiem primārās aprūpes speciālistiem ir jāapzinās sava loma mutes veselības veicināšanā un jāsaņem atbilstoša apmācība (Gambhir et al., 2019). Pilotprojekta ietvaros tika aptaujāti 14 praktizējoši ģimenes ārsti, tai skaitā 3 praktizējoši ģimenes ārsti-rezidenti. Galvenie rezultāti - puse pētījuma dalībnieki savas zināšanas par bērnu mutes veselību novērtēja kā labas, aptuveni viena trešdaļa kā vidējas un viena septītā kā sliktas. To var izskaidrot ar to, ka pamatstudiju un pēcdiploma studiju laikā lielākā daļa ieguva nepietiekamas zināšanas vai neieguva vispār. Vairāk nekā trīs ceturtdaļas uzskata, ka ģimenes ārstiem būtu jāiesaistās bērnu mutes veselības veicināšanā. Pētījuma hipotēze daļēji apstiprinās, jo respondentu iesaiste bērnu mutes veselības veicināšanā ir laba (lielākā daļa veic mutes dobuma izmeklēšanu, nosūta pie speciālista, ārstē gļotādas bojājumus), bet zināšanas par bērnu mutes veselību ir vidējas un vājas, jo vairākums respondentu nevarēja pareizi norādīt bērna vecumam atbilstošu rekomendēto fluorīdu saturošas zobu pastas daudzumu.
Primary care medical professionals, especially physicians, can significantly improve the oral health of their patients as part of general healthcare (Tadin & Dzaja, 2023). Family doctors and pediatricians see newborns and children more often than dentists, so they mostly have the first contact with the child and parents and the opportunity to educate about the child’s oral health, prevention and treatment of oral diseases (Tadin & Dzaja, 2023). The objectives of the work were to create a validated questionnaire ‘’Family doctors knowledge and involvement in promoting children’s oral health’’ and to investigate family doctors knowledge and involvement in promoting children’s oral health. In Latvia, the latest information on the prevalence and intensity of caries in 12 year old children was collected in 2021 and the results showed that D1MFT= 98,5% (D1 is enamel caries without cavity; D- decayed, M- missing, F- filled, T-teeth), D5MFT= 71,9% (D5 is dentine cavitation) (Maldupa et al., 2021). The incidence of childhood caries is still high in Latvia. Untreated caries in a child of any age or early childhood caries negatively affects the child’s well-being and health - body weight (nutritional problems, difficulty eating, chewing), growth, development, communication and grades at school (Tadin & Dzaja, 2023; Uribe et al., 2021). Family doctors and other primary care professionals need to be aware of their role in promoting oral health and receive appropriate training (Gambhir et al., 2019). In the pilot project, 14 practicing family doctors were surveyed, including 3 practicing residents-family doctors. Main results - half of the study participants rate their knowledge of children’s oral health as good, about one third as average and one seventh as poor. This can be explained by the fact that most of them acquired insufficient knowledge or none during their undergraduate and postgraduate studies. More than three quarters believe that family doctors should be involved in promoting children’s oral health. The study hypothesis is partially confirmed, because respondents involvement in promoting children’s oral health is good (most of them perform oral examination, refer to specialists, treat mucosal lesions), but their knowledge about children’s oral health is average and weak because most respondents incorrectly stated the recommended amount and concentration of fluoride toothpaste for children.
